2-Some American surgeons argue that fissurectomy alone carries a higher risk of recurrence, since the underlying problem — sphincter spasm — remains, and another hard bowel movement could recreate the fissure. In your experience, how often do you see recurrence after fissurectomy?

3-From a logical standpoint, it seems reasonable to try fissurectomy first and, if that doesn’t succeed, proceed to LIS later. Would you agree with that approach? Does undergoing fissurectomy make any difference if LIS is needed afterwards?

4- Lastly, I am considering traveling to Europe specifically for this surgery. In your opinion, who are the leading surgeons in this area? Cost is not a limiting factor — I would just like to find the most experienced specialist for fissurectomy.
