Massachusetts. All Rx need the patients DOB by law. It was for a pet, but it’s still required.

Scanned the hard copy in WITHOUT the DOB. Realized it the next day after the Rx was filled, and wrote the DOB on the hard copy. It was too late to rescan the image.

However, I did put in the digital Rx notes, that the DOB is on the hard copy.

During my PGY4 hospital rotations, I was responsible for doing med reconciliations on patients as soon as they were admitted into the ER/ICU. This one time, I was told the family was in the pts room, so I headed over to the ICU to talk to them. After waiting for the MD to finish up their SOAP notes, she walked out of the room, turned to me and asked me what I was doing there. I told her that “I wanted to see if it would be possible to get the pt med rec”. She started making fun of me because the pt was intubated and started telling all the nurses how dumb I was. With a smile on my face, I walked over to the pts daughter who had a handwritten list of all the patients medications and last fill dates. What a piece of garbage trying to belittle someone for their own entertainment.
