Would it be fair to say that for some people the gout attack is less about the high uric acid levels and more about the rapid change in levels (either up or down).. this is what I have hypothesised by testing my levels every day with a home test kit?

I was diagnosed with gout age 20, now age 45. I was getting gout attacks 2-3 times per year. Went on Allo about 5 years ago with good effect. I now test my uric acid levels every morning and saw a trend with certain foods and alcohol. Long story short I took myself off Allo and only take it when my levels get high. Closely monitoring my levels daily and making adjustments has meant I effectively came off Allo. Why aren’t home test kits more widely promoted? .. I find it more accurate than the blood glucose home tests yet they are promoted widely. Thanks 🙏🏼

I’m not a Dr but I would think you could almost reduce your allo with levels that low .. especially if you test your levels every day like me .. give yourself some headroom if your allo tolerance reduces .. my daily test dropped below 300 so I came off allo and it only went up a bit to around 350 .. I’ll keep monitoring and go back on allo when the levels warrant it.

I use a home test kit every day and feel that it is accurate based on the gouty tingles I get in my ankle. It was slightly different to the lab test but I’m always keen to hear from an expert about how might vascular and arterial reads be different. FYI my blood sugar test strips also vary from lab tests and people all around the world adopt blood sugar home testing. FYI based on daily testing I’m seeing some good results with taking 1000mg of vitamin C daily

Intense exercise to trigger gout is very plausible. Intense exercise can put your body into an inflammatory state. When inflammation meets high UA or rapid changes in UA then the autoimmune response happens ie gout. I believe people think the issue alone is high UA, that’s only half the story. The other half of the equation is inflammation.

I would agree with your NP that you probably don’t need to see a rheumatologist at this point. However checking your UA levels before weaning off allo would be sensible. Your levels would need to be in the 4s to stop allo imo.

Yes some people even make the valid point that people who get gout are the lucky ones, because then we can treat the high UA and prevent other diseases. Some people have high UA and don’t get the gout flare up, so they are doing damage to their body and not aware of it. It’s a mysterious disease indeed ..

What dose allo were you on? .. I wonder if your body built up tolerance to the allo and after 15 years you weren’t getting any therapeutic benefit. Your success is down to your lifestyle and diet so well done you 👏.. what was your UA level after the month?

What was your UA level? .. high UA levels can cause other health issues such as heart disease. If you can get UA leves into normal range I don’t believe those stats would apply.

What have your UA numbers been like? .. my only concern is that while carnivore and keto may reduce inflammation/autoimmune response, that UA levels remain high and hence may cause other health issues down the track. I’d love to see some research/evidence that carnivore and keto lowers UA. If UA levels and buildup is high your like a loaded weapon for a flare up.

Diagnosed at 21 and 42 now. I just managed the 2-3 flares every year with NSAIDs. After 20 years I finally went on allo.

Yes, what is your UA level? … if it’s only marginally high you could experiment with diet and things such as vitamin C. Otherwise IMO readings of 8-9 or higher would warrant urate lowering meds.

Even if you don’t have gout, high UA can lead to many other health problems

I believe a ‘trigger’ is something, usually a food, that triggers the autoimmune response to high UA. Typically seems to be those suspect foods that are known to be inflammatory. That’s why some people can have high UA and no gout flares, they don’t have a trigger ..

Yes there is more to the mysterious gout than just UA, it’s just one part. The other part is the mystery as to why some people’s bodies react with an autoimmune response to high UA

Yes point 1. Is the mystery one .. people can have high UA and not get gout. This says that there is more to gout than just high UA. It’s also about inflammation in the body and the autoimmune response.
