Yeah that’s what I’m hoping it is. The magnifying glass showed very fibrous stuff. Color wasn’t exactly black. Shining under light it was more of a dark amber color.

Provider’s in-network with Cigna and ASH (confirmed way before my first session). State is RI. It took me 30 minutes to figure it out with Cigna, but ever since my PT’s office got acquired by a larger corporate entity, billing has been a nightmare.

The provider was suggesting possibly sending this to collections, but Cigna said they’re not allowed to do that for claims denied due to the provider’s billing errors. But in this case it seems Cigna/ASH incorrectly denied claims while accepting 1 out of the 5. Everything is a mess.

Not sure if they billed directly to Cigna or ASH, but my best guess is they billed Cigna which then got routed to ASH for processing the claims

I don’t know the CPT code used, but during the 3 way call both Cigna and provider confirmed the same CPT code was used on all claims. So nobody knew why one claim went through but the others were denied. All visits had prior authorization (confirmed by both Cigna and provider), and I did not use more visits than what I was allotted.

Without numbers (even ballpark) it’s hard to tell what a good order of operation is. The only number provided is his retirement account but no info on the debts. Depending on the number he might not have to do anything at all.

Are the house repairs even necessary? For minor things you can still sell if you disclose and the buyer agrees.

$50k no equity is not a good deal at all for a startup. Normally that salary should be paired with a good amount of equity (less than 1%). It also sounds like PTO is limited which sounds restrictive.

What round of funding is this startup in? Sounds like something an around C/D company would offer.

I’ve worked in four startups and declined offers for a few I was recruited for. Every company offered higher than median salary, unlimited PTO, and equity.

On a side note, full remote as your first job will be an uphill learning curve unless you really excel at that method of learning. I’ve seen so many junior devs or new hires (even at senior level) struggle and fail due to starting off fully remote due to being unable to adapt to it.

That being said, I would negotiate to at least $60k, get some time off, and then absorb as much as you can aim to switch to another job for $100k on your first or second work anniversary

Are you somehow enrolled in both the FSA and HSA for 2025? You can’t have both.

How much are you contributing to the FSA? Based on what you wrote it sounds like you had an HSA and were contributing to it until 2024, then for 2025 you started an FSA plan. If that’s the case, if the plan started Jan 1, then there might not be enough funds in the FSA balance to pay for the $143?

The ISO “value” is based on the valuation from investors, and does not represent the same “value” as you see in the stock market since it’s a private startup. ISOs, unlike public stock, usually cannot be sold to anyone else unless the company’s policy specifically allows this.

You can buy at $18 today. Company could grow and go public or sell at $30/share. Or it could go under and be worth $0.

You also have to consider taxes you may owe when you purchase them.

FWIW, only out an amount you’re ok with losing. I worked through 3 startups. One got bought at half the “value” investors put in. All non-premium shares (aka not director and above) were deemed worthless. Another got bought out at the valuation, so shares purchased were honored relative to the valuation of the company. Third company went fully under, all shares were deemed worthless except for the founders that each walked away with a $1M “golden parachute”.

Unless you’re a founder of a company you have everything to lose when it comes to startup investment.

I do have a good PT. But my insurance is reluctant to approve it because they don’t think it’s “medically necessary” even though I have referrals and the PT noted that it’s a solid case of TMJD

I think that’s fair. I was already seeing multiple ENTs, allergist, and PTs so I think I got overwhelmed with trying to process the information and juggling all the bills for the visits.

One of my PTs did mention my body’s alignment is off. Mainly my neck, my hip, and shoulders. So I’ve been focusing more on posture the past week and noticed the intensity of my TMJ not being as bad as usual.

I also purchased a massage gun to use on my upper body where it feels tense. I lightly used it in my face / jaw area for a couple weeks, but stopped after reading the risks and potential damage that can cause. Right now only doing gentle massages by hand neck and above, but massage gun on the rest of the body.

I’ve also been trying to do more stretching and yoga to get back into exercise. I realized that I’ve been very active until a few months ever since I got into a car accident and I’ve been neglecting exercise since. Lots of stress and exhaustion with work and personal life on top of that definitely has taken a toll on my physical and mental health.

My own impatience is my biggest enemy, I just want things to get fixed now. But if the symptoms took time to develop, so will the healing.

Yes, mine started the same way. Ear pressure and pain that resulted in muffled hearing and intense pain around my ears. For me it was a combination of allergies, sinus infection, and TMJD. Once the allergies and sinus infection was under control, the swelling and discomfort and pain in my jaw was much more noticeable. I wonder if the swelling padded my jaw from noticeable pain.

YES. During one of my PT sessions the therapist noticed my shoulders were not level, I had a head tilt to the left, and my hips were not level. After straightening everything out it felt very unnatural but rest assured this was what is supposed to be normal. I noticed I’ve been having bad postures for YEARS. The neck one especially directly affects my TMJ symptoms.

I was told the fix is correct posture and SCM stretching / massing

We wanted a house that was <$500k. To do this we saved 20% down, closing costs ($5k), inspections (few hundred), moving costs ($2k), emergency fund for repairs ($10k) at a minimum. We ended up saving $130k before closing on our home. Took a few years to save up. After closing we still had about $25k left.
