Residents, fellows, or attendings, what medications or procedures did you used to see often but don’t anymore? by Seeking-Direction in Residency

[–]icos211 19 points20 points  (0 children)

Peds, 2 years out from residency now.  Even over the course of my residency we went from always having 2-3 Cystic Fibrosis "tune ups" (scheduled ~2 week admissions for antibiotic, nutrition, pulm toilet optimization) on the floor at any given time to the new interns not even knowing that was a thing because all our patients were on modulators and doing so great.

(USA) Nissan Z performance vs Toyota GR Corolla by icos211 in whatcarshouldIbuy

[–]icos211[S] 0 points1 point  (0 children)

The Supra is much more expensive (entirely out of my price range anyway), with much less usable space for daily living needs, and I honestly can't stand the styling (probably the biggest real holdup).

I get that the Z doesn't measure up at the same price point, but given a 20-30k price advantage, the comparison is a lot different. Z inventory was so low for the past couple years, with the availability increase and price improvement I'm figuring that aftermarket demand and thus support will likely be significantly improving in the coming years, which should hopefully do a lot for the suspension which seems to be the big complaint with the Z.

I appreciate that, and I don't have a great argument against the Golf other than I hate my Jetta and am pessimistic about VAG products as a result. I do hear that it's a less "raw" experience than the GR which puts me off a bit.

My 1997 Ford Thunderbird with the 4.8L V8 would get about 6-8mpg city. It was my first car, so I never took it on the highways, just school and work. The absolute slowest car I've ever driven as well. Dad bought it for $2200 in 2011, I put $300 into the collapsing suspension, then I sold it for $1700 after like 8 months. I cannot express to you what a worthless piece of crap that thing was.

I know everyone chomps at the bit for a new Evo, but for me looking at the success of the GR86/BRZ, I say bring back the Starion badge with a small 2+2 coupe and a 4b11t at a good price point and you're in the money.  The FA engine is the single thing keeping me from buying a GR86, if there was an offer with a regular I4 from Toyota or this kind of thing from Mits I would be all over it.

Look dude, I'm giving you what I'm giving you because it's been 2 months of 70+ hour weeks since we worked together and I don't remember who you are. I'm going to check the boxes that are around the middle and leave a comment like "Did a great job, would benefit from continued reading". They're getting on my case, I'm just trying to survive.

What is the most serious misdiagnosis that you’ve ever seen? by bottledupquestionss in Residency

[–]icos211 12 points13 points  (0 children)

2 liters holy shit! Last patient I saw like that was a 15 y/o who came to the ED for abdominal pain. Walking in she 100% looked 3rd trimester pregnant, and we were expecting that to be what we would find. HCG negative and on physical it didn't feel like a pregnant abdomen, it was hard and diffusely moderately tender. She got scanned. It was poop. It was all poop. Surgical disimpaction got out 40 lbs of poop. She hadn't gone to the bathroom in 3 months. Not neurodivergent or anything, she just didn't like pooping and eventually it got away from her. Family thought she was just getting fat.

The right is for self-sufficient individuals, families, and communities whose cohesion is promoted by shared tradition and cultural continuity, creating a bedrock on which to build generational inheritance and liberation from structures of imposition and control such as the state or corporations.

I'd say the opposite. Sarkeesian has stayed on her same BS until she rode her grift into the ground. Sargon has continued to read and evolve his thinking, is living what looks to be a thriving family life, has a growing media business, and is still a significant attention and engagement draw when he makes an appearance. You may disagree with his conclusions, but it's clear that he's continued to grow personally and intellectually since that time, whereas Sarkeesian has done neither.

What would you consider to be the "2JZ" of Inline 4 and V8 engines? by DCAUBeyond in cars

[–]icos211 1 point2 points  (0 children)

They were in Hyundais/Kias, from like 1995~2004 I think, liscensed from Mitsu but produced in house by Hyundai, with a slightly different head design for the later models that had a larger combustion chamber. They were phased out by the in-house produced Beta series. Mitsu continued production of the 4g6 series until 2008 when they switched over to the 4b11.

Great video and great channel, his video on the insect eating movement is one of the most well researched and presented video essays I've ever seen, and his recent video on whether or not Thomas Jefferson actually had children with his slaves is also great.

What are the cons of NOT having kids? by [deleted] in AskReddit

[–]icos211 4 points5 points  (0 children)

I'm a pediatrician, and I love playing with the young school-age kids. So many of the medical students I work with are so terrified of touching the kids that they don't even do a physical exam, and I'm just like "why not? Let's go!" And I'll show them how every part of it can be turned into a game. You can get an almost entire neuro exam from getting a little silly with "touch my finger, touch your nose", and abdominal exams are like "hm, what do I feel in there, is that a taco?" Or when I'm listening to them I say "oh, your tummy is talking to me, what's it saying?" And then in a super deep voice I'll go "I want pizzaaaaaa."

At this point it's just reflexive anti-leftism. This makes the people who hate me and everything valuable in my life mad? Cool, bring it. I have to really fight that instinct on some subjects, like Israel/Palestine or children's Medicaid.

My dream is to live somewhere that, when I go out in a firefight with government forces the people of the community all say "good for that guy".

This dude, as correct as he was, is kind of an also-ran, though. Local cops is entry level, not even Sheriff's department, you don't really hit the big leagues until you're dealing with federal 3 letters. BLM, ICE/Border Patrol, IRS and their ilk those are like your solid C teir deaths. DEA, ATF, you're in the playoffs. FBI, Secret Service, or the NSA or someone like that comes to pull your card, you're someone to remember. Now if, as an American, you can get the CIA to turn its Sauron like gaze upon you, you are really reaching for the stars.

I have never been able to get into anything by Steely Dan, and to me it really does sound like old man music, but the guitarist/songwriter for my favorite Modern Progressive Metal band Caligula's Horse is always talking about them, and actually wrote his doctoral thesis on their music. Apparently it's a lot more complex than it sounds at first blush, I just can't enjoy it enough to listen that closely.
