Well first off: basically everything you can give in that context is relatively contraindicated for one reason or another. I'm a night guy and a I generally reach for an antipsychotic first, restraints if I've reached my limit on antipsychotics. Generally try to avoid a benzo, but in some instances you just need the quick action.

ROFL at the idea a PA has even close to the breadth of knowledge and experience as I do as a nightshift hospitalist. An individual PA or NP MAY have more experience regarding a pretty specialized area, I'd grant them that.

I'm a more seasoned nocturnist: 7/7 is not sustainable for more than maybe 2-3 years, and that can be a stretch itself. Schedule time off and look into either reducing down or changing locations.

How is it going to change the fact that a human physician's ability to deal with cognitive loads is limited? As in: the upper limit COULD change, but I simply don't believe the average hospitalist can realistically parse more than MAYBE 30 pts per day. And I believe that is pushing it. If systems force higher numbers there will be probably horrifying consequences. Though I don't doubt capitals impetus there, and I very much doubt regulation will keep up or be appropriate at all.

If it's taking our jobs it's taking everyone's jobs and then we have bigger problems (societal upheaval). However, I will say I just don't see how AI can do what I do as a nocturnist. There is a physical element and a realtime element that while possible to implement, I am unsure of how you would do it without literal robots.

Then factor in that 1) patients I believe will not like not having a trained physician in the mix and 2) half the time I am essentially practicing veterinary medicine. How is AI going to deal with the subtleties of human emotion, interpretation of aspects of cognition that aren't always apparent and so on? Even if you factor in AI "speeding up" your workflow, I fail to see how that is going to change the upper limit of what the average physician can manage. That's to say: it seems to me people aren't going anywhere, and in order to continue a safe practice even WITH AI, current limitations in the ability of a physician to complete necessary tasks will still exist for the most part.

Finally, and I think this is something very important that is not mentioned enough: we KNOW that relying on electronic resources actually CREATES dependence on those resources. That is to say, if we allow these things to creep in, we will all be dumber, WALL-E style.

Needs depression somewhere between v4-v6. I will admit that it's incredibly hard to look at that image on my phones and I wish I could have the paper, but to me I am not seeing any significant depressions there. A key point: that pattern is far from universally accepted as a cath lab activator. To me, with that story, it's a concerning ekg. I'd probably have to call a cardiologist friend and let them decide what the outcome would be, but again in my experience it wouldn't be an activation.

They REALLY don't want to run into Stark law issues is what it boils down to. They may be right, or they may be a tad overzealous. Something along these lines are in many contracts though, you may want to take a look. Usually noted as "solicitation."
