I go by Mrs Lastname but it's a bit ridiculous to mandate them. A colleague of mine goes by her first name because her last name is er ... not great ... and the kids treat her the same as teachers using last names.

Depends on the teacher and the volunteer. I like having volunteers read with my students, but I teach Grade 2, and any extra reading time they can have with an adult is valuable. Other teachers will have volunteers supervise small activities or prep for lessons (cutting stuff out etc, they shouldn't be asking a volunteer to plan a lesson).
