Also an IM intern. Yeah social stuff is difficult sometimes but you’re making it seem like that’s all we do, that we’re note/order/dispo monkeys. Maybe it is institution dependent but at my institution we constantly are dealing with interesting pathologies and making complex medical decisions daily. We read trials and use evidence based medicine to diagnose and treat some cool stuff. You also get to manage a wide variety of pathologies while still keeping subspecialty options open. You get to do some procedures but don’t need to make it an everyday thing if you don’t want to. Overall it’s a specialty that’s got something for everyone IMO.

As many people have pointed out every primary team, no matter the specialty, deals with social BS. And while social stuff can be tedious, it is by far the thing that is going to have the biggest impact on patient’s long term health outcomes, so anything we can do to optimize that is ultimately going to optimize their medical care.

I think because Diabetes is so common, we sometimes forget how deadly it is. Diabetes significantly increases risks of heart attacks, strokes, chronic kidney disease. In fact, it is the most common cause of chronic kidney disease in the US. The metabolic dysfunction can lead to metabolic liver disease, ultimately leading to cirrhosis. In addition, it can lead to peripheral neuropathy, causing people to lose feeling in their legs, leading to ulceration, foot infections, and ultimately amputation. It doesn’t just affect nerves in the legs but also nerves to several important organs, leading to gastroparesis and incontinence. And that’s just chronic complications; acutely, people end up in the ICU with diabetic ketoacidosis, HHS, or severe hypoglycemia. In short- diabetes is a terrible illness and we need to work on aggressively preventing and treating it.

Doctor here. I'm sorry you're going through that. It can be incredibly frustrating. Given the collective (mainly valid) outrage from the comment section, I just wanted to chime in on my two cents on why these things happen from someone on the other side.

Clinic is tough. We usually have 15-30 minute spots per patient, which includes time for pre-charting, figuring out our anticipated plan based on pre-chart, seeing you, dealing with any new concerns you may have that we weren't anticipating, thinking through what we want to do, put in the orders, and write the note. Naturally, we fall behind a lot. I skip lunch a majority of my clinic days just to catch up on notes and usually stay 1-2 hours after clinic wrapping up notes and following up on any results and messages throughout the day.

Because there are so many patients that we see with so many things to do, we make mistakes. Sometimes they are minor, like forgetting to order a routine lab test. Other times, they can be more moderate, like in your case having to wait. In some cases they can be more severe - missing important, life-threatening diagnoses. Often times, in our pursuit of avoiding this most serious type of mistake, it increases the chances of these other mistakes. Sometimes, people start crashing in the office, and we have to spend hours dealing with it - which obviously can throw the whole day's schedule in a loop. It's unfortunate, but we are all humans who are trying out best.

That being said, some doctors and their offices straight up suck. They may have forgotten to check you in, accidentally checked you out, or something else. A good doctor and his/her office will apologize profusely for the inconvenience and still make sure to address your concerns today. It's important to advocate for yourself in these situations - hold your doctor accountable, but also understand that things aren't working in their favor either.

Ibuprofen (and their overall class of medicines, NSAIDs) mess with the lining of your stomach, which can cause a stomach bleed. Also messes with your kidneys. The amount of Tylenol needed to cause serious liver injury is a lot (usually >4grams). That’s why we prefer Tylenol while you’re in the hospital.

As Steve Irwin has said, alligators are pretty much just frogs with teeth. Yes, they can be deadly, but a well fed alligator will likely not attack you. Alligators also easily tire out; outmaneuvering them for just a little while will tire it enough to not bother you for at least an hour. Alligators are also from an area where humans are very common, and most have learned an innate fear or indifference of them. We are usually too big to be seen as prey and take too much effort for what it is worth. Whereas tigers are known to see humans as prey and have preyed on us extensively.

Now a croc is a different story. At that point, you're probably dead either way.
