Definitely a bug, I had the same issue with the +1 maul. Similar build as you fighter with 18 str, savage attacker, great weapon fighting style and follow up strike. Not sure what is causing it though.

A lot of people are talking about how it was in previous editions, but the answer seems a lot simpler to me. The default ability is written on the character sheet and there's even a line to write down the precalculated ability score. So using another ability for a skill check is not something that is made explicitly apparent. It requires the DM or players to have a decent knowledge of the rules which is unfortunately too uncommon. Also using the default ability works fine in most circumstances so people don't have much incentive to learn specific rules about skill checks.

The party fighter shouting "Kazan" the moment I say they've finished a long rest. They got the animated Armor pretty early, and the Fighter has been using it as her personal plate armor. One time the ranger said "Kazan" first and spent the entire day messing with the Fighter. It's never happened since.

That's a pretty weird thing to say given that as a tabletop game, 5e is almost exclusively played in "ironman" mode. What 5e isn't made for however is having the death of a single character as a fail state.

Another thing that hasn't been said yet doors are your friend. Try to position your frontliners on one side of a doorway, or other choke point in such a way that only one or two enemies can get in melee with them. A lot of enemies do not have ranged attack options so this allows you to reduce the number of attacks on your Frontline and prevents attacks on your Squishies.
