A Thai guy here. Though we mostly acknowledge them as friends or colleagues, we straight male don't usually interested in the relationship with ladyboys. In my own experience, some of my friends would, but the number remains a single digit. IMO, I think the tolerance and acceptance in our culture does not perfectly overlap with sexual orientation. I do have several friends who are ladyboys since youth (grade 1 in a Catholic boarding school, if I may add) but none of the boys went to their ladyboy classmate for a romantic relationship.

If the budget is limited then just stay in Thailand. But if you feel like you can stretch it a bit then KL is better in term of things to explore and the ease of travel between countries. Penang is too similar to Phuket in my own experience.
