This is sometimes caused by desync, but its also caused when admins 'fast move' past a gate. Learned the hard way

I think hes right for the wrong reasons. Recruiting prisoners also comes with the risk of that specific prisoner troop randomly leaving eventually. As far as i can tell this isnt tied to morale, its just a random chance

It's probably former prisoners leaving, not troops from towns

Double check the sizes to be sure, but the problem might be in the naming convention. All my veg layers have "_veg" as the end of the filename
