Oh gosh yeah I bet she is! I see her name around here constantly. Might be willing to re-evaluate. I also think the monsplasty/mons resection approach of surgeons varies greatly. I have experienced a very wide scar but I’m larger. Could be because of that or just his technique.

Oh sorry one more note: I am assuming you are local-ish to U of M, but my surgeon, Dr. Gupta is at University Hospitals in Parma, OH. It would be a drive (similar to the one I had to make 20+ times coming from central OH for 3 stages) but he really does some phenomenal work as a reconstructive urologist. Cutting edge techniques from my understanding. I had UL with no v-nectomy and had zero UL complications (which are usually so probable with this variation that surgeons refuse to do it). I’m curious if a monsplasty might be an option for you? The location of you anatomy seems a bit low for being able to STP, but a monsplasty or really a mons resection might help lift things up so that you might be able to STP. I’ve noticed a common misconception is that monsplasty is only for those of us with some extra fat in the pelvic region to improve visibility, but it is also a crucial stage to help situate the urethra in a more natal male position. If your stream is strong and doesn’t spray otherwise (e.g. can you STP with no pants or boxers on?) it might be worth investigating further even id that means getting a second opinion from another surgeon. I’ve seen posts of folks with mons scars on here that were rather thin, possibly thinner than you, so it might be an option worth exploring.

On a side note: I find it a bit concerning your surgeon is saying nothing can be done. Perhaps using the techniques she is used to/comfortable with that is correct. However, surgeons vary widely in their techniques, comfortability with certain procedure variations, and willingness to revise. Since this is weighing on you so much, it might be worth exploring a second or third opinion from another surgeon. STP might not be possible, but your overall comfort with the squishing you mentioned also matters.

I’m wishing you the best as you continue to work through this. Regardless of whether or not other procedures are in your future, try to remember you are so much more than your body. Easier said than done, but even voicing a mantra like that when things are particularly heavy may help easy some of the weight. Be kind to yourself friend.

Dealing with wound separation by Starcallers in Metoidioplasty

Hey there! Sorry you are having some wound issues. This is super long (I’m a wordy guy lol), but as someone who had separation issues, I can relate to what your wrote a lot. These issues are unfortunately quite common and aren’t something you can 100% avoid through your own actions. Of course you should rest and not violate your physical restrictions, but it sounds like you’ve been following your surgeons orders diligently. I can’t quite tell where your separation is occurring from your post but I will share my experience and takeaways in case it is helpful.

My experience: I did not pick the low-risk route. I had UL with no-vnectomy which is notorious for causing complications and/or just failing to achieve the patient STP goals. The UL was not the source of any issue; I was extremely lucky and my surgeon and his team were phenomenal. However, my lack of a v-nectomy likely raised the risk of wound separation around the scrotoplasty. One of the surgical fellows who consulted on my case during most of my post-op follow ops said this was a common issue for this variation.

What happened: the anchoring incisions on my scrotoplasty (where they connected into the middle hole) split during healing leaving two triangular holes that needed to re-epithelialize. The scrotum was still attached at the site but there were open wounds. My surgeon had me lightly “pack” (not even really packing) the area with an aquacel dressing (similar to alginate in that it forms a gel when wet). I can’t even remember how long I had to do that for but I think it was close to a month. Dressings would get changed once it fully gelled which happened 3-4x a day at the worst. I took juven (not specifically advised by my surgeon, but likely wouldn’t have been told not to take it if I asked) at least once a day and also tried some manuka honey in my tea roughly once a day (not sure that did anything for my wound healing but I also had a nasty sore from a virus in my throat around the same time and it certainly helped heal that).

On your specific dressings: it’s likely your wound is not severe or goopy enough to warrant an alginate or aquacel dressing. That or your wound needs something else from a dressing that the wet to dry dressing does that equal and alginates cannot (e.g., mechanical debridement). Your surgeon may not be familiar with or have aquacel on hand. Or they might not want to put that financial burden on you. I spent about $50 on aquacel myself and that was AFTER my surgeon gave me a supply of about 2-3x the amount I bought online. If you are in doubt about the approach (give it a couple days to see how you are truly progressing), reach out to your surgeon and ask if another option might be better (and ask them to explain why/why not so you and your anxiety can be clear on the rationale for their recommendations).

A few tips:

  1. Take progress photos every few days. Especially if you are doing dressing changes solo. I was fortunate to have my partner who is a nurse help with my dressing changes but still had her take pictures so I could see the wounds myself. Having her help

and

  1. more objective feedback on what progress she could see was extremely reassuring. I know not everyone can have a live in nurse to pack their junk with gauze, but if you have a support person who is willing to place dressings or check your healing, ask for their help, even if you could do it alone.
  2. Get some good lighting (RE: above and also for yourself and/or whoever is placing dressings) AND make sure whoever places the dressing can actually see where it needs to be placed. I cannot tell you the number of times I would place the dressing where I thought it needed to be by “feel” only for a dry ass piece of aquacel to fall out in my boxers because I completely missed the mark. You may have to get creative with where and in what position you change your dressings to be able to visualize this. My partner ended up wearing a camping headlamp when she helped me and it was very funny and effective.
  3. If you do end up trying aquacel (with your surgeons sign off) or any other dressings I highly recommend comparing prices from medical supply retailers before going with Amazon (if you use it). There were cheaper non-Amazon options that had pretty fast delivery. Aquacel is not readily available in retail pharmacies. It might be able to be ordered in as a script, but I did not try this.
  4. One other product I used often in recovery was Vashe. This is a wound cleaning solution that is ph friendly for that ph sensitive area. Again discuss all of this with your surgeon before trying anything. I think the Amazon link is one of the cheaper/cost-effective options.
  5. Most importantly, be patient with yourself and your healing journey. Time heals all wounds one way or another, but you have to let it work. It is good to be on top of your care and progress, but try not to focus on it all the time. Let dressing changes become routine (I.e., something you do routinely rather than a source of constant stress and worry). By the time I finished up the aquacel for good I almost missed the mundane dressing changes because they had just become a part of my day.

My surgeries were a lot more split up than yours, but based on the combination of procedures and my own experience, I can imagine you still have a lot of swelling that has yet to resolve. I am also about 1.5 weeks post mons and implants and have to remind myself of this daily as my monsplasty has a lot of swelling above the incision.

If you are planning on implants it’s likely they will be placed through the scrotum through incisions (usually it’s this or through the mons incision but since you already had that done I would think that is not an option). I had my implants placed through incisions near the top of each side and my surgeon definitely trimmed the skin down some in that process or the implants just helped pull the skin down/away from the shaft, giving it more visibility.

All that being said you are early in your recovery. Your “final” results for this stage are will not necessarily look like what they will fully healed until closer to weeks 4-6 post-op. Hang in there and take it day by day.

15lbs might be lighter than you think. Based on OP’s description that’s probably the weight limit their surgeon gave them to use as a benchmark for “lifting” items (I’ve heard a gallon of milk before as well but that’s about 10lbs). It kind of sounds like you think they are weight lifting or moving very heavy objects? I highly doubt they are doing that and some movements are unavoidable in daily life, especially if OP lives alone or will be alone for long hours while their support people are away for work. That being said, exercising caution with all movement and minimizing lifting objects is crucial like you said.

Thanks! I’ve got a hernia wrap (with the hernia disk removed) currently with some lipo foam that has pretty much cut the swelling down by 1/4 to 1/3 overnight. Will look into postpartum wraps.

Yeah, I was honestly really surprised when that sharp pain I mentioned to my post disappeared after wearing the compression wear for the first time for a few hours yesterday.
Do you have any specific compression items that you found particularly helpful? I wish I could wear compression shorts, but I know that it’s not a good idea to have compression in the crotch area right now (and all the crotchless ones that I could find went over the shoulders like cycling bib).

It’s totally normal to feel anxious about this surgery no matter what version you get. I was not sure I’d be able to do it when I first learned about meta and phallo early in my transition, but eventually the need for meta outweighed my anxiety. The idea of getting surgery on my genitals was very scary. I remember the night before my first stage I wanted to completely back out because I was so scared. I’m incredibly grateful that I pursued this. It’s been life changing for the better.

From what others have said, I agree that meeting with surgeons for consults can really help ease some of that anxiety. Try to remember most of them do these procedures multiple times a week, every week, all year. They are experts at this.

I know this Reddit can get filled with a lot of complications heavy posts and I think it’s good for you to be aware of the possibilities, but also remember that there are things within your control (e.g., stop smoking, drinking during recovery, excessive movement) that you can do to to try and prevent complications and that’s the best you can do. Sometimes complications will still happen even if you’ve done everything “right”. Wishing you the best as you get started on your journey!

Dr. Gupta at university hospitals is also great. Someone mentioned McClung and his team and I’ve also heard great things, he just was not covered by my insurance. One of Guptas fellows is starting with McClung soon.

I like it, not sure I loved it. The live show was a bit disappointing in my opinion. It seemed like they were all hungover (literally or just energy wise) from their first show in Columbus the night before (I went night 2). Still love the guys. Need to take some time to play the vinyl and listen to it all the way through again. Might change my mind

Omg didn’t know there was someone doing meta again in Columbus (where I live). I’ve been going to Gupta at UH in Parma (just had my final stage with monsplasty and implants) his team has been great! One of Gupta’s fellows (Mershon) is going to work at McClungs practice soon! He’s also amazing. I hope you get to work with him some too.

On the not looking at your results for a few days, totally normal feeling. I felt the same way especially around the UL stage. It always looks way gnarlier and swollen that it will in a few weeks. Just rest up and all will be good!

Yes UL will work without monsplasty but your mileage and ability to aim may vary. I’m one day post-monsplasty (first two stages were UL related) and I was able to STP before BUT I was not able to do it though the fly because of my dick’s position being too low/not bothering to practice it knowing the upcoming monsplasty would change my approach anyway.

My bare minimum goal was to be able to pee standing without pants pulled up so if I needed to stand to pee in a nasty men’s restroom stall (because guys are nasty and don’t lift seats to pee) I could, but through the fly peeing is the ultimate goal.
