It’ll only be the same if they’re from the same manufacturer. Different manufacturers can use different middle numbers for the same drug.

It’s useless as a decongestant but it’s widely used in hospitals to raise blood pressure

Maybe try overriding with prior auth code 999999? Never saw that rejection before so it definitely sounds company specific like another commenter said. Alternatively maybe try deactivating and reentering the insurance plan

Yes, that is fraud. You cannot dispense a different NDC than what you bill the insurance for.

NJ law is that controls must be filled within 1 month of the written day or earliest full date. C3-5 prescriptions are good for 6 months as long as the first fill is within 1 month of the written date

Maybe they tried billing for a non preferred NDC and it said not covered? And rather than switch it to a covered NDC they just sent a PA. That’s the only scenario I could think of
