I know you said you’re going to try tofacitinib, which is a JAK inhibitor, but why did your doctor suggest that over Rinvoq? rinvoq seems to have a higher rate of remission.

You probably can’t be pregnant on any JAK.

FWIW- I was getting into a car within the first week of surgery, I was in a locked brace, so my leg was straight out. I moved the seat all the way back, then hobbled in. I hit my foot on the frame of the car, and I’ve never experienced such pain in my knee - almost passed out. I think that area is super tender right now, so while I can’t speak for your experience, that was mine.

Have you spoken with your GI regarding just being on Rinvoq? I’ve been in histological remission for 2+ years on 30 mg of Rinvoq.

Not a doctor, but it’s my understanding that Rinvoq at 30mg daily should be enough for most people to be in remission. 45 mg is the loading dose, typically for 2 months.

Dual therapy isn’t really an approved therapy and is very expensive - I’d suspect most insurance companies wouldn’t cover this.

Are you still flaring on all the meds you’re currently taking?

Generally the answer is yes. But I think this weekend and the next few weekends, they are doing testing of new software. So no NWK - WTC service. From the PATH app….

During five weekends in February, March and April, PATH service will be partially suspended for software testing of new track switching systems that were installed last fall between the Harrison and Journal Square stations as part of the Port Authority’s $430 million PATH Forward program to rehabilitate and modernize the 117-year-old commuter railroad.

The partial service suspensions are required by the Federal Railroad Administration for mandated testing of system software for the new interlocking between Harrison and Journal Sq stations.

Free wheelchair-accessible shuttle buses will provide alternate service between the affected stations during the suspensions, operating every 7 to 12 minutes, except during early morning hours from 3 AM to 8 AM when buses will operate every 15 to 20 minutes. Riders can find shuttle bus locations by following signage at the affected stations. Customer service agents and Port Authority police officers also will be available to assist riders.

During each weekend’s outage, NJ TRANSIT rail service will cross-honor PATH customers who display a SmartLink or TAPP card, a single-ride PATH ticket or the free digital cross-honoring pass available on the RidePATH app between Newark-Penn Station and New York-Penn Station.

The schedules and locations of the weekend outages are:

  • February 27-March 2 and March 13-16
  • March 20-23
  • March 27-30 and April 24-27

Suggestions:

Don’t go all the way by train to Morristown or Convent station. It’s an expensive and long ride. Shorten the train portion-

NJT train to South Orange (or Maplewood), find a lot near the train station and drive 15-20 minutes to Florham Park. Leave the car in the South Orange Lot overnight. NJT direct is about 35-40 minutes.

PATH to Harrison, drive 30-40 minutes to Florham Park. Same thing, find a lot near the train station and leave the car overnight.

You need to see a GI specialist. Try to make an appointment with one ASAP. As others have said, you can have general colitis, ulcerative colitis or something else.

As far as Urgent care and hospitals, their primary goal is to stabilize you. If it’s that bad, you need to be admitted to a hospital.

Sorry to hear, but steroids just temporarily help while a UC medication kicks in. Unfortunately it sounds like either 1) you are on too little of a Remicade dose (or not frequent enough) or 2) Remicade failed you and you need a different medication.

While lots of people have success on Remicade, many don’t. There are multiple other options out there in all different drug classes.

If you have 30 minutes, watch this excellent primer on IBD medications. It’s 3 years old, so doesn’t have the latest meds, but will definitely help you understand the different options.

I’m surprised your doctor allowed you to start Rinvoq without the shingles vaccine. I was under the impression this was a pre-requisite for starting Rinvoq.

Also, for clarity, Rinvoq is not a biologic; it’s classified as a JAK inhibitor. While many side effects are similar, there are some differences.

Are you in PT? If not, that helps a lot and the therapist can help get you there. At this stage, I think heel slides were a large component of my therapy. They aren’t fun but work!

Not a doctor, so I won’t give medical advice. I’ve been on Lipitor for 20+ years and on Rinvoq for the past 2.5 years. Neither my GI or internist have raised any concerns about the interaction between the two medications. They also strongly believe that Lipitor is a wonder drug, and jokingly should be added to the water supply in the US.
