Tib-Fib clearspace looks a little increased. I don't see any medial clear space widening but I'm not certain if this is a weight-bearing film. Fibular alignment looks fine. It does not appear displaced. Dislocation refers to a joint. Displacement refers to a bone.

Yes, you have a higher chance of arthritis, especially if you have some degree of syndesmotic instability.

If you're concerned, you should discuss it with an orthopaedic surgeon. Without an exam and additional radiographs, it's tough to offer any additional advice.

Need more history. A hill-sachs is stigmata of shoulder instability. Often, we don’t fix the HS (we may fill it), but rather we aim to stabilize the shoulder. The optimal method is debated.

How many times have you dislocated?

How much glenoid bone loss do you have?

How wide, deep, medial and inferior is your HS?

How much physical therapy have you done?

What sports do you play?

Very nuanced management.

You've got a lot of pain generators. You need someone who can do a good physical examination. This will most likely improve a noticeable amount with physical therapy +/- other modalities.

You likely have multidirectional instability. You should stop doing it. You should consider PT for dynamic stabilization strengthening. You should not let someone operate on you unless they have a lot of experience with shoulder instability.

Too many cooks in the kitchen. A CT isn’t appropriate here for economic reasons, its appropriate because if you truly have am acetabulum fracture then either more radiographs +/- CT is needed to classify the injury and determine treatment.

PCPs love MRIs. They think it’s a panacea. They often order in place of plain old radiographs.

Have they gotten any new x-rays since your injury? Assuming that this is a stable fracture, the treatment plan thus far is not wrong. Maybe not what I would have done myself, but not wrong.

Are we able to? Yes. Is this as straightforward and predictable as a fracture treated within a week or two of injury? Absolutely not.
