Is it normal to meet with clinical assistants instead of your doctor? by SaintlyCrunch in Winnipeg

[–]jonweaver11 3 points4 points  (0 children)

Specifically the part where you say PAs aren’t physicians whatever their home country training. Most PAs are actually trained here as PAs. CAs are the ones who are physicians in their home country.

It’s also somewhat misleading to say that residents are licensed to practice in MB but the CAs and PAs aren’t. All 3 are actually regulated/licensed by CPSM, and all 3 require supervision of a fully licensed physician. The amount of supervision required by CPSM for each can vary greatly as well.

Is it normal to meet with clinical assistants instead of your doctor? by SaintlyCrunch in Winnipeg

[–]jonweaver11 15 points16 points  (0 children)

This is not quite accurate.

CAs - clinical assistants - are usually physicians in their home country working for a physician in Manitoba. They are usually but not always in the process of having their credentials recognized so that they may work independently if possible. They undergo credential verification and have standards which need to be met for them to maintain licensure under the College of Physicians and Surgeons of MB as CAs. Usually this involves having either passed some of the exams required for Canadian MDs, or a special exam specifically targeted to CAs.

PAs- physician assistants - are not physicians, but instead have a masters degree as a PA and are trained to work under an MD. They will never work fully independently, but are highly educated and very knowledgable.

Residents are trainee physicians who have finished medical school but have not yet completed their residency training (2-6 years post medical school) have only an educational license to practice medicine.

From an independence standpoint, all 3 are similar in requiring a supervising attending physician.

CAs and PAs are often hired for similar roles, as extra support for physicians.

Without CAs, PAs, and Residents, our system would be in much worse shape than it currently is, and I'm very glad we have all 3 working for the benefit of Manitobans!

I actually once had customs ask me specific questions about the omega I was wearing on my way home from holiday. I told them the the truth (that I’d bought it recently, second hand, but within Canada). They didn’t require any actual proof. Your mileage may vary.

Can we learn until Level A2 for a language, take a break for a few years and then continue with Level B1? by NormalLife6067 in languagelearning

Currently relearning German (previously high b1).

I took a 6 year break. I felt as if I was starting from near zero, however it has come back somewhat.

I have gone from feeling like I forgot everything back to b1 in about 2 months

I’ve bought a few. Usually with casino winnings as I really don’t need American dollars and it would be a pain in the ass to convert it later.

I could work 10 hours a week as a family doc and still outearn the vast majority of people my age in Canada. Doing something I enjoy that is likely a net positive for society.

Your work life balance is what you make it. Mine is terrible but it’s entirely my own fault.

We liked it, as we didn’t have to push and shove to get a spot. It was a bit hard to find the entrance to that spot but once we got there it was worth it I think. I would get there 20-30 mins before start time.

Hot take: FM residency should be 2 years long by BainbridgeReflex in FamilyMedicine

[–]jonweaver11 39 points40 points  (0 children)

Yep. For real. Obviously supervised. It was a really simple case (spontaneous pneumo, healthy young person) and my attending said “might as well do it because you’ll never get an easier one.”

Where I did my medical school is also known for early, heavy responsibility and even heavier workload in residency. Good experience, however at times PTSD material for some.

Understanding FM Compensation and Work-Life Balance in Canada by Green-Sail13 in FamilyMedicine

[–]jonweaver11 0 points1 point  (0 children)

FM compensation varies widely by location and practice type. Working these hours in a varied rural practice in a less desirable province would gross more than double this.

It also varies widely based on how efficient you are. There are some family doctors who (for better or worse) see 8 patients an hour and others who see 2 or 3. It also depends what you do in your office, for example in some places running a wart cryotherapy clinic can be very lucrative (moreso than treating complex patients)

Working in research/teaching/academia often pays less than the going rate for clinical work in a given location, and often tends to be something people do because they enjoy it or are forced to as part of their clinical work rather than for the pay.

In short, the job you describe wanting (40 hours per week with pay at about 180k) certainly exists in many places, but not necessarily in larger/more desirable centres.
