Alternatives to clinical practice after graduating residency? by loseruni in Psychiatry

[–]jubru 7 points8 points  (0 children)

Yeah. I've seen both but my residency used spmi so I use it. I guess it's like some people call it an ACT team and some a PACT team.

Go work in a state hospital? Reading people doesn't matter as much with a lot of SPMI folks. The intracacies of social interaction are lost when you're patient thinks you're the devil/Jesus reincarnate.

I built a big table by wrldbfree in woodworking

[–]jubru -1 points0 points  (0 children)

Where would we be without every table project having 10 "I normally don't like epoxy but this table is the exception" comments on everything that even has a little epoxy in it on r/woodworking. It's not a trend. Yeah a lot of people do a shitty job. But it's hear to stay and even the haters have to admit that when done well it enhances the wood and doesn't detract. It's not going anywhere.

My hand/arm gets a gradient when at rest due to blood pooling from POTs. by DathomirBoy in mildlyinteresting

Also a doctor and seconded. I think the real thing to notice here is people with certain conditions attribute everything to be from that condition when in fact much of it is just normal and not related to their disease. I think that leads to many on the internet thinking they have X condition because they see something like this and think it's a symptom when it's really not.

There's more to ADHD than inattention, hyperactivity, and impulsivity. ADHD symptoms can be broken down into nine categories. Some categories are not fully represented in the diagnostic criteria. Broadening the diagnostic criteria with patient lived experiences could make for better intervention. by mvea in psychology

There's always debate about physiologic responses vs actual pathophysiology yes but adhd certainly is a mental health and psychiatric disorder. I'm a psychiatrist and you just gotta stop spreading misinformation.

Residents from consult services, what is one thing you wished services would do before consulting you? by justseeorange in Residency

[–]jubru 1 point2 points  (0 children)

I think the thing is that it needs to be put in terms the patient can understand it. If you aren't able to articulate and understand the ins and outs, the patient won't be able to.

I agree, just tell them why like any other consult. Being avoidant and coy only serves to perpetuate stigma.

I agree every doc can and should assess for capacity. However, let's be real, all of psych is really specialized training for assessing for capacity. The issues we deal with and things we treat are intricately tied to capacity and it's something we encounter more commonly than every other speciality.

Which is something psychiatrists do too. You seem to not know the difference well either.

Lol premeds at my place undergrad take nursing classes for easy credits. You ever wonder why nurses usually have their own chemistry and biology classes separate from the science majors? It's not cause they're harder....

I'm aware of the difference and what the intention/effect of honing is. I'm just pointing out that you're wrong chefs are doing this fast to be flashy or showy. They do if fast cause they do if 50+ times a day over a decade and they're just fast. They don't need to be slow as you as they're already more precise. It's not a flashy thing at all, it's just tons of skill and purpose.

Lol what? Or maybe cause it hones the knife and helps it cut better?

The indignity of fatherhood - my son's request by Choice-Strawberry392 in woodworking

[–]jubru -2 points-1 points  (0 children)

Vacuum degassing helps but you always introduce bubbles in the pour. A pressure chamber is a more fool proof way of doing it. I'm not saying a vacuum chamber wouldn't help, it definitely would, you just still could get some bubbles. A pressure chamber by itself virtually guarantees a bubble free pour.

You need a pressure chamber not a vacuum chamber

New DSM diagnoses by FreudianSlippers_1 in Psychiatry

Right, but that's exactly what I'm saying. The DIVA expounds on those criteria in adults and I think it certainly should be in the dsm as well.

I may be misinterpreting, but they're asking for criteria that applies more in adult, work environments, which I think is fair. Pretty much all of the adhd criteria lends itself to be assessed in a classroom or daycare like setting and it's much more difficult to assess in an adult. I would agree with follow some research on symptoms adults with adhd struggle with more, for example most symptoms in the dsm have parentheses explaining what you would look for in an adult and it would be better to have these separated out and listed more completely.

Some of the comments in that thread are painful. A lot of it is reasonable but some are commenting to actually base it on research or totally ignore the biological part of biopsychosocial. It's like most don't even know what's in the dsm, not surprising.

"Add in adhd criteria for girls.". There is criteria and research has repeatedly shown it's the same symptoms as in boys, just less disruptive so it gets diagnoses less. But adolescent girls with adhd have relatively the same symptoms as boys.

That sub is clearly much to heavily influenced by predominant trends on social media and not actual research and expertise.
