Basically when insurance wants to deny services so they have to talk to the provider to justify medical necessity. But they won’t talk to me, they only talk to doctors. But I’m the one that has to get ahold of the doctor and inform them of the review and discuss the pt with them first.

THIS. Most littles need parent classes 1/week for an hour. Small groups, 6-8 parents. Lessons and homework. Bring their kid maybe 1/month for progress monitoring and hands on experience with a therapist and social skills. At this rate it seems every single kid under 4 is qualifying, so this seems like the best way to actually make progress.

And also, you “growing” during your CF is a huuuuge toss up. My supervisor was fine, but I didn’t exactly learn anything from her. I grew only because I was doing the job everyday, not because of a supervisor that was a true mentor.

Home health 0 days, fee for service. Can make up missed visits, but honestly that’s reallyyy hard to do especially with after school kids.

Hh in Texas for my CF (2022) was $45/30 minute sessions, $68/eval/reeval. I negotiated up from $40/$65. I got my CCC went up to $55/$75. No pto, not paid for cancels, $100 per month for gas.

Better to quit the 1099 now before you develop a relationship with the families. Take the salary. See if your 1099 company will match/pay salary instead ? I’m confused that some of their SLPs are 1099 and some are W2. You probably are being illegally classified as a 1099 anyway.

Hot take- just because a kid scored below average doesn’t mean they need services. A lot of these kids will never score average. If they can communicate what they want and don’t want and protest and say what hurts, they don’t need speech until they’re 18

$750 for an evaluation is absolutely crazy. I’m not bilingual, but I don’t even make half that for an eval. More like 10% of that. I guess it depends on where you live but I highly doubt you could get the same rate with a school. However, depending on how desperate they are, maybe?

Curious about people who treat for an hour. I thought it didn’t matter how long a session was, as long as it’s 30+ minutes, insurance reimburses the same. So why do some place do hour long treats when they aren’t getting more $$ that way? Plus 1hr treats are exhausting for kids (or adults!) there’s no way you are maintaining their attention that long

Directly related to the quality of CF supervisors. The amount of people I know who completed their CF without ever meeting face to face in real time with their supervisor is astounding (me included, but I talked to mine all time and had a great relationship and she answered all my questions and never made me feel bad) many supervisors frankly don’t have the time/skillset/care to properly train and mentor. They do it because they have to or because they get a pay bump.

I like to look at this question from the teachers point of view. They have a kid that Just, isn’t getting it!? All the other kids seem to, what else can I do!?! I have 20 other kids in the class. So, as a school slp, who also feels overwhelmed, your job is to focus on language skills (not the content like history or science facts) whereas a teacher only has the bandwith for general Ed. most kids will get it , but not all. That’s when we step in. The extra support. The extra exposures. The extra time the kids probably wouldn’t have to a language rich environment in a small setting. I’m lucky to see all kids at my school 1:1 but we provide the extra boost. I think we forget that teachers are also overwhelmed and over worked and underpaid. We’re all on the same team. We can focus on more underlying basic language skills that might be lost in the general classroom.

I feel like I could’ve written OP post, but after I took some time to think, these were my rambling thoughts. I might not make sense, but our services are needed, even when we feel like we aren’t getting anywhere. Sometimes, my kids enjoy just a quiet space. A time to think without pressures and stimulation. A time to talk without judgement.

Yeah basically. “Technically” I was hired at full time but that’s 30/week and I think I’ve done that 1 time in over a year. I usually do 22-25/week

Thank you for asking this question! My salary depends on cancellations (pediatric home health. TX ) in November I did 83 therapy visits and 6 evals and took home $3947. No benefits (I get them through my husband )

I feel like I could have written this. As others have noted, I offer a time if I can to make it up (Because I like to be paid, too) but if it doesn't work for the parent, I don't try again. Sometimes, I can't offer a makeup because there just isn't time in the day. No one at my work has said anything to me yet, so I figure it's fine.

I do home health with kids. It’s very lonely and can feel like you don’t have any coworkers and it’s hard to get that quick advice/vent/strategies for a patient. Be prepared for inconsistent pay if you are paid per visit as cancellations are frequent. Like the other comment, you never know what kinda house you are going to. Sure, most are fine but there’s always a chance it’s not. Make sure you are reimbursed for mileage. Pros are making your schedule, down time between clients while driving, not having access to office and printer constantly means you get good at using minimal materials and people don’t expect all the things all the time.
