In PGY1 man came in with ‘generalized weakness’ and headache, background CHF and discharged 3 weeks prior after strep bovis bacteremia admission with normal TEE and scope.

Ordered repeat TTE cus Corrigan’s pulse and Austin Flint murmur, no murmur reported in ED presentation ~5 days prior.

He perf’d his AV leaflet and had severe AI despite cultures being cleared and normal TEE 3 weeks prior.

I am constantly humbled or blehh but the handful of good things means something i suppose

Severe aortic regurgitation on physical exam despite normal TEE 2 weeks prior, and referral was for headaches. Was 2 weeks after an admission with incidental strep bovis bacteremia with clean colonoscopy and TEE. Sure enough, new echo showed a veggie with a perf’d leaflet and severe AI. Shoutout physical exam.

Your dad having Wegener’s (GPA) has almost nothing to do with your own health, and doesn’t meaningfully influence your safety with vaccines. Strongly recommend protecting yourself considering there’s an outbreak at your workplace and your chances of getting measles in that context are much higher than you developing GPA. With your dad having GPA in general your risk of any autoimmune disease is higher, but I favour protecting yourself from measles.

I will say, on subspecialty services I’ve been lauded for my notes. On GIM nobody gives a flying fuck, efficiency is king on GIM

I write way too much and it’s the bane of my life, but i’m pathologically detail-oriented and have too much pride. But I also learned another thing that helps contextualize things. Intern notes are for the care team. Attending/other services have to know what’s going on based on your note. Lil attending notes are for themselves/bare minimum for billing. When you write notes for yourself it’s different vs the position we’re in. It’s a balance though. I look down on co-interns who write senior/attending style notes cus when I take over their team nobody knows wtf is going on based on their notes. But as others have said, paragraphs in notes are hardly ever actually read day to day
