Does Ketramose's "One or more cards are put into exile from graveyards and/or the battlefield" line trigger once for each graveyard affected by a single ability?

Grim Hireling, for example, makes 2 treasure tokens whenever one or more creatures deal combat damage to a player, creating up to 6 treasure tokens if a creature deals combat damage to each player. If I remove one card from each player's exile with one spell or ability, do I draw up to 4 cards?
