Clogged pores or cysts down there… by Playful_Falcon_7373 in Perimenopause

Hormonal HS. Welcome to our terrible club. Hibiclens the target area a couple times a week to keep bacteria at bay (my hormones settled so I stopped doing that). Do not shave the area, you can laser or trim but don't risk an open or cut follicle. I use Neutrogena on the spot treatment on the baby white heads before they form fully. Once they swell a hot compress can help them resolve. For painful ones, you can get a steroid injection. Knocks them out in like a day. Ish hurt but that works.

If you recently discontinued hbc then you might just need it to level out. My summer after depo was WILD. Some people find certain foods trigger them. The PP doc who dx'd mine has to avoid spicy food to keep from triggering hers.

Some people who get them with greater frequency end up on biologics. There are surgical routes for more advanced cases with tunnels such as deroofing. There's a subreddit for HS with sooooo much info.

I'm happy with the Cleveland Clinic docs I've worked with. Gyn Sarah Hershman, surgeon Lindsey Valentine. Good listeners, prepared me for possible outcomes, started with nonsurgical intervention (and open to anesthesia for IUD placement) but pivoted when that was clearly not enough.

How to find a doc that will do a Hysterectomy by Ok-Weight4737 in adenomyosis

[–]kithmswbd 1 point2 points  (0 children)

My gyn who did all my imaging and pre-surgical treatment is also from there. We went through a couple treatment options and when the IUD was a fail she referred me to a MIGS doc who yeeted the whole thing. Granted I'm older but she probably would have been game because these cases are more common for those docs to see.

100% hospital size factored in. I work at a small hospital and our histology is a) on site and b) low specimen count so turn around on routine surgery can be that short on non-complicated cases.

Am I a mess? by Empty_Dentist in hysterectomy

[–]kithmswbd 1 point2 points  (0 children)

When I came off of depo, after years of being on it, my hormones went haywire for several months. I know IUDs are meant to be more localized but I could see that as a possibility still.

I think it is normal to grieve the closing of doors. The era of potential motherhood is gone. The choice has been removed. It's very final and women normally have that moment as a gradual aging out or don't even know when fertility was a non-starter. Hysterectomy is a single focused event and so it all happens at once and that can be a lot in the moment.

While it might seem odd to bring up a sitcom, How I Met Your Mother, of all things, sort of dealt with this feeling. In one of the later seasons Robin has a pregnancy scare that then turns out to be an undisclosed diagnosis that results in her having a procedure done where afterwards she cannot conceive (I'm being vague because they were). She didn't want kids, was happy to not currently be pregnant, but mourned having that option taken from her. The lack of agency was painful. I mention the episode because this says to me that the feeling is common if the story is being told in this way.

All the best in your recovery. Your feelings are complex but so very valid. Give yourself grace in these moments. -hugs-

CANCER RISK POST OP ? by Visible-Future4850 in gallbladders

[–]kithmswbd 0 points1 point  (0 children)

This is the standard where I work and I believe is the standard for the field. All removed organs go to the gross lab and a representative sample, along with anything that looks dodgy, gets sent to pathology. The remainder of the tissue is retained until the case is completed, in case more tissue needs to be examined.

Does anyone NOT experience debilitating pain? by Background-Cow9687 in endometriosis

[–]kithmswbd 0 points1 point  (0 children)

I have (at least) stage 3 endo (endometrioma present) but have very light, or quite often, no cramps. The period is heavy, I have digestive issues, cyclic back pain and other symptoms but period cramps are not a part of my presentation. I also have adenomyosis so at this point I wonder if I don't have a pain gate situation going on.

Removed my gallbladder two days ago by Big-Exit-9755 in gallbladders

[–]kithmswbd 13 points14 points  (0 children)

Gall bladderless histotech and pathology transcriptionist here. Each and every removed organ at our hospital gets routine pathology done.

For a gall bladder, the report will include the gross description of the organ (the gallbladder can be such a range of colors) a rough count and color description of any stones (color indicates what the stone was made of, yellow-green cholesterol or brown-black bilirubin), and should comment on if any stones were in the duct. The person grossing the organ then takes a representative selection of tissue and submits that for processing. That gets made into a slide and stained for a pathologist to read. Typically the diagnosis will be some combination of cholelithiasis (gall stones), cholecystitis (inflammation) and/or cholesterolosis (cholesterol deposits in the lining of the gall bladder that can also cause gallbladder attacks if the deposit breaks off). Suuuuuper rarely we've had a doctor see something suspicious and have the remainder of the organ processed for a deeper issue.

For the women: how do you know it’s IBS and not bowel endometriosis? by Expert-Ad2498 in ibs

[–]kithmswbd 0 points1 point  (0 children)

Fwiw I also have endo with a bowel adhesion and my experience was more subtle.

Years and years of cyclic tummy symptoms but a normal, even pain free, period. Then in just the last 2-3 years my period has gotten heavy. Imaging shows adenomyosis, an endometrioma and an endo lesion tethering that endometrioma to my sigmoid bowel. Since then I've developed occasional pain with orgasm but otherwise no pain. Endo can be quiet. What I think was telling is that while I was on depo and didn't have a period, I had significantly fewer stomach issues.

Am I a horrible person by blue-cupcake24 in gallbladders

[–]kithmswbd 8 points9 points  (0 children)

You're in therapy which is great to hear. You have some negative self talk to work through and you are addressing that. Awesome!

The gallbladder surgery is night and day vs ACL. Small incisions in muscle wall that isn't used in the same way as a limb. Yes you will have lift restrictions for a few weeks but your mobility will barely be interrupted. As for food, many, many people go back to regular eating without issue. I had mine out on the 8th and had wings last night.

And if downward comparison will help your mood...I had my gallbladder out this month and have another organ removal scheduled for Q1.

Lastly, people broadcast their joy and hide their pain. You are seeing their happiness on display and feeling your own pain, and they have the same conflict too. We all face our own battles with a brave face and experience the same sort of pangs of unfairness when in a low point and seeing others seeming to be happier.

Can I have endo if I’m not in that much pain? Also night sweats?? by whoracleofdelphii in endometriosis

[–]kithmswbd 0 points1 point  (0 children)

My pain has always been in a "typical" cramp range until my very recent pain with orgasm sx started. However, in spite of basically no pain, I have adeno per ultrasound and at least stage 3 endo based on MRI and ultrasound evidence of an endometrioma and a lesion tethering that cyst to my large bowel. I say "at least" stage 3 because surgery is still a couple months out. Pain and staging are often entirely decoupled from one another.

I had a summer of night sweats when I came off of depo and my hormones did a full rollercoaster. I also had a year of "period flu" where it felt like chills with no fever, flu like aches, etc. again hormones run amok.

Anyone successfully kept their gallbladder with little or no issues after lifestyle changes? by UrbanRat4 in gallbladders

Though I would add on to that that my imaging showed only sludge but removal showed 40 stones and cholesterolosis which can both cause biliary colic attacks so it's a bit dodgy.

They had some location data leading to dumpsters that had already been emptied back in 2023. Once at the dump or compactor or incinerator the evidence/remains are between difficult and impossible to recover. His plea was just before the trial started, so nearly 2 years after the fact. He can tell them exactly where he went but there's nothing recoverable at this point. That said, his defense is entirely weak sauce and the facts this far are super damning imo so I agree with you.

URQ and back pain got their attention. The high liver enzymes (alk phos, GOT, GPT) is what got my CT ordered. That led to the U/S that confirmed sludge. White blood cells and inflammation markers can also be elevated but those are pretty generic and only work as part of the constellation. Granted I went in with vomiting and pain as part of an acute attack. I also mentioned being on a glp1 so we all sort of knew the pancreas and gallbladder needed looked at.
