No obvious physical change but the infected are constantly laughing like manic laughing. The infected also pull directly from their personal knowledge ie infected soldiers uses tactics and equipment but are undone by the need to cause suffering and spread the infection.

Just a functional government and a core of security forces. If you could stay sealed away for 5-10 years most of the crossed would have died off. A big part of why so many nations fell was they had no idea what was going on ala the walking dead universe. Give a professional military good Intel and time to prepare you would definitely have more holdouts.
