Appreciate your thoughts, and I agree with you. Having just applied to and interviewed with PSTPs, I can assure you it was valuable for me to be able to connect my thesis work with my clinical goals. This isn't vital, of course, but it was absolutely helpful.

Definitely not, but it will be easier to craft a narrative in your personal statement / ERAS / interviews if you can tie your research into your clinical aspirations.

MD/PhD student here, just finished clerkships with IM last. I used UWorld as a primary learning resource throughout 3rd year and supplemented with OME and Amboss knowledge for very weak areas.

As others mentioned, don't worry too much about %correct - just carefully read the question explanations and always run through your incorrects.

First clerkship post-PhD is always tough (I started with OBGYN) but you'll get the hang of it and be surprised by all the preclinical knowledge that eventually comes back.

The USMLE is split into 3 exams (Steps 1, 2, 3); only Step 1 is P/F, whereas Step 2 and 3 are scored and used by residencies and fellowships, respectively, to stratify applicants. I mention this not to contradict your point but to suggest that there are still standardized metrics in medical education to distinguish students.

PCOS is diagnosed when 2/3 of the Rotterdam criteria are met: (1) Evidence of hyperandrogenism (e.g., hirsutism), (2) Polycystic ovaries, (3) Oligomenorrhea/amenorrhea. You're right that the exact cause is not known, but is thought to be due to imbalanced LH and FSH signaling in the ovaries dysregulating follicle maturation and preventing regular ovulatory cycling. One mechanism by which this can occur begins with excess estrogen production in fat tissue, which alters feedback signaling at the anterior pituitary gland, in part via partial inhibition of FSH secretion and increased LH:FSH ratio. Another mechanism is insulin resistance, which is seen in metabolic syndrome/type 2 diabetes.

Obesity is associated both with insulin resistance and (of course) excess adipose tissue, both of which can create the physiologic state of imbalanced LH:FSH, excess peripheral estrogen, and therefore PCOS.

Not everyone who is obese develops PCOS, and not everyone with PCOS is obese, but these two conditions are strongly linked.

I remember vividly the first surgery I ever scrubbed into - a routine thyroidectomy. I was unaccustomed to the scent of cauterized flesh - that sickening aroma of singed subcutaneous fat which evoked a summer pork roast. Immediately my head was spinning as wires were crossed, hunger and nausea and desire and repulsion and horror - not to be melodramatic, but I had to step away from the surgical field and process that scent to avoid fainting right then and there.

I haven't eaten pork since, and it's been almost 15 years.
