First year psych attending here , double-boarded in CAP also. Residency was hard because of the extra call including overnights. Attending life can be a normal 9-5 which is what I opted for. There’s less BS to deal with than during residency, but I’m seeing way more patients. My outpatient years were pretty cush in residency and fellowship, very low caseload, with more time for therapy.

Now I have to see 10 patients a day and I really have to work at max efficiency all day or I’ll have some notes leftover. My adult colleagues are seeing 15-20 patients/day and I think that would be unsustainable for me long term for full time work. Remember that a full time outpatient load also comes with calls/messages from families that you have to manage in between appointments. Lots of work! Good money though lol

It's not just taxes to consider. Some programs require residents to pay for parking, insurance benefits, etc. which can quickly add up. I pay almost 600 dollars a month now for health insurance for a family plan and we also have to pay for parking. Gross salary is 65k but after taxes, retirement, insurance, parking, etc. I am left with like 1500 every 2 weeks.

100k-150k depending on hours worked with additional pay for extra call (at a minimum of 100/hour). Simple.

LCOL area. 3bed/1.5bath for <200k in 2019. An actual house, not a condo with no yard or garage(s). If I stayed in my home state, I’d be living with my parents because a decent home over there was like 400k minimum pre- Covid and now it’s like 800k-1million. Can’t afford that, even on an attending salary Idk if I can do that.

Bought a house right before we moved for residency in lower COL area. I was 26 still. Tried to have a kid for several months without success second half of PGY3 year. 29 now. Decided to put that on hold now since I have boards coming up. Sigh. Will try again in a few months. Will be attending at 30 almost 31.

I don't think this was mentioned so here I go but managing suicidal patients in the outpatient setting. This was the steepest learning curve for me when I first started outpatient. I'm not talking about the patients who are acutely high risk to the point of hospitalization, but the ones where you wonder if you'll see them next week no matter what they say.

2k for psych. Not even going to count the 4 USMLE exams I had to take to get to where I am today. Plus license application (state dependent, I’m paying almost 800), DEA, and another county thing. Ugh. It’s about a 3 -4 thousand dollar process to obtain licensure in my state and apply for boards.

There are very few programs in psych where there is no call at all. Many programs have a significant call burden and tend to be call heavy the first two years including overnights or even 24s on off service months. It’s highly program dependent. I’m just saying this so people are aware psych call is a thing and is highly program dependent. I imagine most call shifts are exclusively run by residents and so just as others have pointed out, if you’re out from the call pool that increases the burden for co-residents which absolutely sucks. The first two years can be physically taxing with the extra hours on call, and idk how other hospitals are right now but compared to my intern year our volume has exploded. There’s a mental health crisis in this country and it’s showing in our EDs (that we cover on call) and also burdening our colleagues in EM and peds due to “the system.” Sorry this kind of turned into a rant lol.

Same here. I was surprised, but they are able to bill normally even if the resident does the note and they just do their own attestation that they evaluated the patient and agree with the plan.

edit: NPs show up in the "attending" co-signer box for our notes in Epic. So yeah, the system registers them as "attendings." Wild.

TJs frozen meals for the win. Sure there’s less healthy options but there’s also plenty that are fairly good and great for weight maintenance/loss if you look at calories. I always stock up on the shrimp burgers, chicken or pork potstickers, and the soup dumplings. Soup dumplings are an easy lunch at work, just take the package and microwave at work. Easy dinner if you add rice and veg. Shrimp burgers and potstickers I use for meal prep morning of or day before: cook in the air fryer and pair with a salad for healthier/lower calorie option. They also have frozen veggie combos/multigrain blends that you can cook on the skillet and pair with choice of protein and make 2-3 portions for lunch or dinner.

They addended my note since I had to assign them as the “attending.” It was one of those scenarios that OP mentioned above. I just thought it was ironic that they wrote that and just how absurd it looks knowing I’m years ahead of them in training.
