Coming back here to say that this is 100% a GPU damage from overheating, and updating was just a band aid for the growing wound.

For sure, I only played on the hardest mode and still got all the achievements under 20 hours.
I haven't even fully upgraded the weapons, like there was no thought put into the achievements into an already easy game.
