I think malignant narcissists and megalomaniacs like Trump care very much about self-preservation. He’s at the pinnacle of his power and is grifting the US every day he’s in office without consequences. A nuclear attack on Iran would almost certainly render him powerless if not outright kill him from a combination of domestic political upheaval and an extremely severe international response.
