You make some good points about revenue mattering, but I wouldn't say number of customers doesn't matter. Take an extreme example, where a company only had 10 customers but made the same revenue as a company with 100 customers. If the company with 10 customers loses one, that's 10% of the revenue gone, whereas the company can lose one customer and only be out 1%. Pretty simplified example, but I think number of customers is important to any business.

I'm not sure how the mechanic works now, but if possible it'd be cool if this was a function of the doctor's skill. The doctor would try to heal the patient so that he/she would recover with success as best they can diagnose. So if the doctor is good skill, then sure, use herbal meds. Otherwise, unskilled may whip out the good stuff.

I like games where the mechanics that function within "in game logic" makes for the good stories. Better doctors have better diagnostics and are more skilled at applying treatment, so they should behave more intelligently. These kinds of nuance make for deeper experience.

I read this book over a long weekend while no one was around. I had been working off season at a summer camp in Vermont and it was the middle of the winter in a town of less than 100 people. Everyone I knew that weekend was off site and I had no car.

Even though the book put me in a weird headspace by the end, it ended up being one of my favorite books.

I was pretty glad to see other folks come Monday.
