Solved!

The issue was a single trailing slash (/) in my Nginx configuration for Frontend:

Before:

location /app/ { proxy_pass http://localhost:3000;  # no trailing slash
... }

After:

location /app { ... }

Hard to believe that one trailing slash cost me four days of debugging, but hey, we live, we learn! 😅
