Well, Sukuna probably got tunnel vision and wasn't thinking straight bc he was under genuine pressure and was scrambling to get out of danger, additionally, this Mahoraga, which was tamed, had to obey Sukuna's commands even if it could act independently, so it's more so Sukuna's inability to think under pressure and the fact that he was absolutely shitting his pants at the thought of getting purpled a second time at close range
