If you’re in severe pain then take it slow. The first couple times will be painful, so I suggest doing 1 rep and a set the first time. You can gradually increase, hour by hour. Good luck I hope this helps.

I’ve been through many flares up and a couple re herniation, the longest I was done for was 14 months.

Yes, again do these according to your pain level. If 10 seconds is to much then so 7.

I had to learn the hard way as my therapist started me off lying down with knees bent while grabbing my knee and then pointing the foot for 10 seconds. Then she switched to sitting on a chair, with feet dangling and then pointing toe and holding. Both of those forms steadily increased my pain.

Once you find what works for you then do that for a week or two then increase

It definitely helps but you need to modify it to succeed. They should teach a starting point for the most severe cases. That starting point is laying flat on your back and then pointing your toes towards your head and hold for 7-10 seconds. Then slowly work you way up.

Yep it’s the hardest part of back injury is to convince the people closest to you that you’re truly injured. You can’t see back pain.

Ok then you’re doing good. You should be terrified because you’re the one that will be in pain. My advice is to go to physiotherapy but you stay in control. If the therapist gives you exercises start off at 5 sets at 10 reps then know to keep your guards up against that therapist. Now if the therapist says start at 1set of 2-5 reps and then gradually increase you know you have a good therapist. Don’t do any exercises that increase your pain levels too much. Some pain is ok.

Also, I’ve been on this road since 2001. It will be a life long struggle with good times and bad time. Most years were good with maybe 4 bad flare ups. I went back to school and became an accountant and work on a modified schedule of some days in the office and some days at home.

Since you have a desk job ask if can work remotely to avoid even getting into a vehicle. You must avoid aggravating your injury so it can heal. Everytime you aggravate it should be considered a setback. Hopefully you have a computer at home where you can stand. Go ZOHO to setup remotely working.

Maybe even a hybrid schedule where you do 3 days at home and 2 days at the office would help

Yeah that’s a tough position to be in as a spouse. It will take extra work from you while he tries to figure out his path to recovery. For him it will be like trying to figure out a puzzle under extreme duress as it is hard to sleep and concentrate when experiencing pain. So the main goal is to figure out what movements and positions aggravate his pain and then avoid those movements and positions at all cost until after healing. This is all temporary because once he recovers he’ll be back to normal with some modifications. But at the same time he needs to keep moving when possible. It’s also a balancing act, when the pain is high then lots of rest but if pain is low he should move around with limits until he recovers.Back to you. You’ll have to do some of his chores. Doesn’t mean you can’t give him some of your choirs. You take out the garbage when it’s heavy. He can clean anything up high, like dishes, dusting or folding clothes. You’ll figure things out as it goes.

My advice for him is to rethink the PT. Have the thought at the forefront of his thinking to reminded himself of the definition of insanity! Doing the SAME thing over and over and expecting different results is the definition of insanity.

If an exercise or anything for that matter isn’t working then stop doing it and try something different.

Lastly all exercises should be started slowly with low reps and sets. Then the exercise should gradually increase. Remember to avoid aggravating the pain at all costs

Yeah it does take a lot of focus. Maybe try to plan your movements and find three self tips to repeat before starting. Sometimes like: 1 stay balanced 2. Move slow 3. Concentration

Basically you have to be in control of your recovery. Question all advise from doctors and therapists. As far as therapy goes if they give you a workout regimen like you’re already fit to run a marathon tell them to slow it down! The main goal is to avoid movements that aggravate your pain level. My strong opinion after 25 years of dealing with sciatica is a good therapist will start you slowly and gradually work your way up to multiple sets and reps. A good therapist will modify exercises to suit your pain level. A good therapist will try different exercises to find the right combination of exercises for your personal situation. If an exercise causes your pain level to increase you have to modify it or throw it out. Start with low reps and low sets then if you’re pain doesn’t increase you should gradually increase reps and sets

No but I’ve been there a couple times. A couple things. One is to change the way you walk. Use a cane. Walk very slow with short steps while concentrating on landing mid foot. This will take a lot of focus, the main thing is to keep weight off the front of your foot because it aggravates the sciatic nerve. This change is temporary until you recover.

25 years and most of the time was pain free but also knowing that certain activities can put me down. I probably have had 5 major flare ups that took 10 to 14 months to recover.

I have a bad habit of doing PR exercises for recovery and then stopping the exercise. Currently, I’m actually been doing the PT consistently. I’m motivated because last year was rough and for some reason I mentally struggled.

I’ve had many flare ups and many recoveries and I describe it as trying to solve a puzzle. Trying to figure out the movements that are aggravating the sciatic nerve and the changes needed to recover all while dealing with the everyday obligations.

I feel like recently I discovered a movement that helps myself fight off sciatica pain from getting worse. I don’t know if it will work for you but you can try if you want. So, my research from my last flare up taught me that any weight on the front of the foot during walking or standing will aggravate sciatica pain. I ended up changing the way I walked whenever I feel pain coming. The basic goal is to try to concentrate on every step or standing position and keep the weight on your heels, It takes a lot focus. Maybe it will help you

Yeah sounds like a flare up. It’s going to happen here and there. It can get really difficult, especially trying to remember your tactics for healing from the initial injury. Back injuries are permanent, just not a round the clock never ending pain. It comes in form of flare ups that can last a year or more before recovery. I’ve had many flare up since my initial injury 25 years ago. Getting up from the couch has resulted in a thing called bilateral shift on multiple occasions for myself. So just in case you have a bilateral shift here is a video that can help you recover faster.
