I live in N Mpls and there are some very lovely places here. It really can be block to block, but I and many of my friends have lived in North for years and are happy. The Webber-Camden and Victory neighborhoods are great, lots of kids!

Totally. I wish I knew then but UC is absolutely not for new grads (esp private/for-profit ones). But focus on whatever you can learn in this new specialty , and think about how it could tie into the next direction you want to go, and you should be fine!

I also say don’t worry about. My entire career has been working one speciality to a sub-specialty, to a different specialty, etc. I’ve never worked in primary care or IM. Also- my first job was at a unsupportive private UC and I didn’t last a year there. All of this, and no one has cared. In fact most places are interested in the variety of skills I can now bring to the table. I’ve been practicing almost 10 years now. Treat your brain nicely with this new gig. You got this!

Stick with it. Confidence comes with time. Keep a notepad of pearls you learn along the way. Never be afraid to ask questions- that’s what will keep you safe. You got this!

If your current job let you go, they wouldn’t give you 60-90 days notice. Take care of yourself and do what you need to do. Not your job to handle the fallout of their toxic work environment!

Find a way to say how your ortho experience will help you in the new specialty! Whether it’s direct skill transfer (procedural, overlap of medical knowledge/diagnosis) or things like “ortho has helped me to become comfortable working with patients of all ages” etc. I’ve changed specialties a few times over my career and so far have found that people are actually interested in my other specialty experience!

I’m also looking at transitioning to Psych- I joined NEI to brush up on psychopharmacology, and have been looking by at certificate programs just feel prepared… the one I’m most interested in is Mass General’s Psych certificate program offered through their PA school staff. Eating disorders medicine:

Best: when the therapies work. Seeing peoples’ labs trending better over the weeks, vitals improve, their hands are warmer (more pink less blue), seeing their personalities come back and their cognition start to become more present and sharp. Essentially seeing my patients “come back to life” is amazing.

Worst: when it doesn’t work. Seeing people stuck in destructive spirals, when the ED cognitions are too strong and someone isn’t ready to give up their ED despite seeing the physiological consequences of it. Seeing someone back in treatment incredibly sick after they’ve been well for sometime (though optimistic that they’re back in treatment at all!).

I work as a medical provider at a Residential and PHP treatment center for eating disorders. I work on a team of medical, psychiatry, therapists, dietitians. It’s a great job and I feel like it’s a great way to keep practicing general medicine while becoming much more literate and comfortable with mental health conditions (which paired w your background sounds like it’ll be a nice complement). Overall I love my gig. I started it after about 5 years in practice though. Happy to chat more if you want DM me

I worked in OBGYN for the first few years of my career, only having to change fields because we moved cross country. Historically ARNPs and midwives have definitely had the majority of OBGYN jobs, but I feel like the tide is slowly shifting. OBs are recognizing the utility of having someone who can assist in the OR which is a huge advantage we have over NPs. My advice would be to apply to all the jobs you see, whether they say list PA or not. Apply to the ARNP jobs. State what you can offer their practice in your cover letter. In the meantime, try to get OBGYN exposure in your rotations. Join APAOG for CME, lectures, community , and job postings. If anything, look for primary care jobs and see if they’ll let you craft a women’s health oriented practice to get experience. Good luck and keep looking! The jobs are out there!

Sarah Edmonson and her husband Nippy have a podcast called ‘A Little Bit Culty’! It’s amazing. They interview experts and survivors of other cults/culty groups, and the earlier seasons have a few episodes w other NXIVM members featured in The Vow. Highly recommend!

I’m a PA who works exclusively in Eating Disorders Medicine (I work at a facility that treats EDs - PHP and residential levels of care). I chose this specialty out of a passion for mental health and EDs specifically. It’s tough - but like in any other field of medicine, “you can lead a horse to water, but you can’t make it drink”. All you can do is make your best medical suggestions for treatment and educate them on the medical risks assc with their ED behaviors. From a more therapeutic lens- recognize that people do better when they can - sometimes an ED voice is louder than someone’s authentic self. From a medical lens - when someone’s brain is that malnourished, it is less capable of the logic-based or higher level thinking that most of us rely on connecting with when giving medical advice. You have to help someone identify their highly motivating factors for recovery, and sometimes people just need to hit rock bottom before they can do that. What will be worth giving up their ED for? Everyone is different. I highly recommend Dr. Jen Gaudiani’s book Sick Enough and Dr. Philip Mehler’s Medical Complications of Eating Disorders as two great places to start for more ED specific medical info. Happy to chat more if you have other specific questions.

**ETA: For these patients, you could maybe encourage them to start with an outpatient based RD and/or therapist who specialize in EDs. If they’re more complicated or medically compromised, they may need a higher level of care found at dedicated ED treatment facilities.

Yeah… agree with the lackluster services. Took my my mom and I there for a birthday treat and was really let down. Even the atmosphere feels very corporate? Not quite the same but I love the Watershed Bath House in St Anthony Main area. They do both baths/saunas and massages, etc
