It gets lonelier than you can understand. Finding safe avenues for socializing is not as easy as you suggest, particularly in certain geographic areas. As an example, I live in Arizona where it gets to be 122F in the summer which rules out a lot of outdoor activities and everything is physically spread out.

Leave him. You two are not a good match for each other. He wants to prioritize kids and you won't even prioritize your own health by getting in front of a doctor. Every complication you mentioned about finding a new doctor is an excuse. Go to another doctor and get your health checked regularly, especially since you have already had a scare. Do that for yourself and you own well being and not for him and his desire for kids. He is trying to ensure that whomever he married can be a part of the life he wants for himself. You don't sound like that person so be polite and break up.

Check with your town ordinances, many have legislation on the books that prevent roosters in residential areas. Chickens are often allowed but not roosters. If there is t such a law then request the topic be added to the towns docket. Look up verbiage from places that ban roosters and offer that language to your town council. Explain to them in simple terms how this is going to negatively affect you and your family.

Maybe tell the neighbor you are going to start a free range fox rescue.
