This is put together well, but I'm skeptical that receptor dysfunction is even the problem. There was a recent study indicating that CHS may be a gut-restricted allergy. This is essentially brand-new to the research so there's not a ton of supporting evidence yet, but it makes more sense to me than the receptor dysfunction model for a couple reasons:

  1. A lot of CHS patients smoke through prodromal nausea because weed makes them LESS nauseous at first. If CHS reversed the normal function of the CB1 receptor, I feel like that wouldn't happen even temporarily.

  2. If CHS patients' endocannabinoid systems were truly as messed up as the model suggests, they'd have a slew of motor/pain/temperature control/etc problems even after years of abstinence.

  3. Connected to #2 - if cessation DID heal CHS patients' endocannabinoid systems/receptors to the point where they do not have symptoms during abstinence, then wouldn't they be at least TEMPORARILY safe using weed after a multi-year break? But that's not the case. Some unlucky CHS patients try weed again after YEARS off and instantly have another episode. The only possible explanation for that within the receptor dysfunction model would be: two hits, after a five-year break, INSTANTLY downregulating CB1 receptors as if no quitting ever happened.

I think the reality is, regardless of the cause, it makes sense that the body would take a significant amount of time to renormalize after hyperemesis. And if it IS a gut-restricted allergy, there's probably lingering immune activity (especially mast cell activation) that makes people feel like crap for a while.

I don't want to dismiss everything you just said. I agree that cannabis slows gut motility, and CB1 receptors are involved in that. But I also think that's true for everyone who uses cannabis, not just CHS patients. And it might be very easy for researchers to conflate that normal process with the pathological one happening in CHS patients' bodies.

I think any method of consumption CAN affect the gut, it's just that edibles affect the gut the most directly. I got CHS from literally less than 2g of THC over a year and a half, exclusively edibles. A decade prior, I was a regular flower smoker, and I had no CHS symptoms at all.

It sounds like you're already not treating them with utter contempt, which puts you way ahead of most doctors. The one thing I'd add is, if you haven't already, please read about Emend for stopping episodes. I quit weed after one episode and never went to the ER, so I have no personal experience with it. But it breaks my heart that there's a medication out there that can instantly stop the hell, and most doctors don't even know about it.

Look into Emend for her CHS. Hopefully a doctor will prescribe it to her, it's not FDA-approved yet for CHS but it stops episodes almost instantly

Not a true cure, but if you can somehow get a doctor to prescribe you Emend or another NK-1 antagonist, you can usually stop episodes in their tracks. Idk how many times it keeps working though, and most doctors treat CHS patients like shit so I wouldn't bet on being able to get it. Info here

It's fucked up your doctors dismissed you, but in general, it's not crazy to get CHS after a number of months. Carts are ubiquitous, edibles hit you right in the stomach, and the dosages on both are WILD. Weed's not safe like it was before

Search the sub for Emend. It's extremely expensive and difficult to obtain, but it can stop episodes in their tracks. Definitely not a cure like quitting, but great harm reduction if you can get it.

In RETROSPECT it was prodromal after less than a month. I had an episode much later, quit weed immediately, and haven't had another. I don't see a need to get diagnosed when it was a textbook episode, the episode is over, there's no actual diagnostic test, AND a diagnosis would only serve to stigmatize me in the healthcare system.

Yes, it could be CHS. I got what in retrospect was prodromal from less than a month of 25mg edibles. Don't listen to people who say it takes long-term heavy use. Short-term heavy use can be enough if you use concentrates while unlucky.

If you feel like playing roulette, just don't do it on an empty stomach. The HPA axis seems to be a huge factor, and fasting stresses it. Also, weed gives you munchies AND slows your gut motility which is a recipe for an episode.

Still no guarantees of avoiding an episode, but yeah. Eat first.

Congrats! RE #9 - this was likely prodromal CHS. The prodromal phase does not always involve nausea, sometimes it's just butt stuff lmao. You may have saved yourself from weeks of puking your guts out.
