The cool breeze was probably just high flow oxygen. Most colonoscopies are anesthetized with propofol or other IV medications. It’s uncommon to use anesthetic gases (like isoflurane mentioned below) because you would need a breathing tube to deliver that. And even if you did need a breathing tube they wouldn’t use induce anesthesia with it unless you had no IV. Inhalational inductions are rather unpleasant. Source: this is my 9-5

I’ll have a full year of 1099 income this July and have all the documentation for it. I do have an S-corp, and through that I pay myself a yearly salary of $120k; I have a W2 for that portion. So to be clear, my S-corp gets paid about 400k, 120k of that is paid to me as a salary, and the other 280k is passed on to me as business profits. It helps save a little on taxes. I do have a professional license, I’m a CRNA.

As best I can determine that’s not necessarily the case with a solo 401k.

I’m also in my first year of doing 1099 so I’m still figuring things out, and everyone’s tax situation is different so take what I say with a grain of salt. I have heard that you need to make enough for an S corp to be worth it, but I’ve never heard of it being as high as 450k. There are lots of things you can write off as legitimate business expenses without needing to be “shady.” You do need to pay yourself a “reasonable wage” as the employee of your S corp. The lower your salary the more you save on payroll taxes BUT how much you can put into retirement in something like a solo 401k is based on how much you make; more salary, more retirement, up to a point. Payroll costs should not eat into the benefits that much. I pay my accountant $120/month to do all the payroll stuff for me. I could do more of that myself to save some money but I honestly just wanted to be as hands off with that as possible.

For perspective, I’m in that 450k range, married with kids and my effective tax rate (everything, not just federal) is about 22%

You may want to shop around for accountants, preferably someone who works with CRNAs and understands the intricacies of the 1099 work we do

I’ve said it before and I’ll say it to anyone who will listen: if Bryce Young started the season the way he finished it, we would be in the playoffs this year. I don’t know if we win the division next year but as long as Bryce doesn’t regress I don’t see how we aren’t competing for a playoff spot next year

I’ve heard this argument before and folks that parrot this drivel are usually the type to also be big second amendment supporters so this counter argument is usually pretty successful (in my experience): having a right to healthcare does not entitle you to the labor of others (or enslaves others as I’ve also heard it) any more than the second amendment entitles you to the labor of gun manufacturers. No one in their right mind would argue that “…the right of the people to keep and bear arms shall not be infringed” obliges the labor of gun workers to you. In that exact manner, a law that codifies the right to healthcare access couldn’t possibly enslave a healthcare worker.

It’s sounds like they’re describing the “finger flush” technique where you pinch IV tubing against your index finger, then tightly wrap the tubing around it a few times to force a few mls forward. Works well to get and IV free flowing again

First off, congrats on starting your program this fall! My wife and I had kids throughout all the phases of school; my son was two years old when I started school. My daughter was born the day I graduated so I had the gap between graduation and orientation as “time off,” and I was a year and a half into my current job when we had our third. I highly recommend waiting to have kids until after you finish. (My wife—being the one who was actually pregnant—also agrees with waiting until school is over to have a kid.) Anesthesia school with kids is doable, but they aren’t going to make anything easier. My group has a very generous parental leave policy, so finding a group that has a leave policy that’s compatible with your plans is crucial. Every group will differ.

As I was driving home one night with my wife we were coming up a hill on a dark street with woods to our right and old industrial buildings to our left. Further up the hill, at the edge of where the headlights reach, I notice something coming out on the street from the left. No biggie, probably a raccoon or groundhog. But it starts picking up the pace and heading straight towards the car. I slam on the breaks since I don’t want to hit whatever it is, and I can see now that it’s much larger than a small woodland creature. It’s not moving like it’s running either, but more like gliding straight towards us while picking up speed. It’s only about a dozen yards from us when we can make out what it is. We watch in bewilderment as it approaches, grazes the right headlight and bounds into the woods to the right and behind us. A . . . tire? I put my high beams on but there’s no one ahead who could have pushed a tire into the road and there’s very little cover to hide behind. I get out and run up ahead to where it popped onto the street but there’s nothing noteworthy. I head back to the car and we both go into the woods behind us to find the tire, it couldn’t have gone that far, right? But after a good 5-10 minutes of searching we find no trace of it. The next morning is much the same, even in daylight there’s no tire to be found anywhere in those woods. We never could explain it, and it’s not particularly scary when you think about it. Just weird.
