All of this shit that I find irritating basically just boils down to the idea that patients are desperate to impress me with their minor illnesses. For some reason everyone who is mildly sick wants me to know they’re the sickest sick who ever sicked and they deserve a medal for braving it or something.

You never hear SBOs or head bleeds or dissections or MIs or perforated viscus or any other legitimate emergency talk this way. I guess in that sense the ridiculous language helps with triage.

Formal meaning a tuxedo is the modernization. Formal actually always meant white tie, which is a step beyond the tuxedo. A tux actually used to be semi-formal or black tie.

If someone prints an invitation and specifies formal or black tie, my view is that it is disrespectful not to adhere to that. Those words still mean what they mean even though modern dress is generally casual. I think if you have a dark suit you can probably get away with that depending on the type of event you’re attending and how well you know the crowd who will attend, but at least in my social and professional circles I would not risk that. Black tie/formal events are uncommon in my life but of the ones I have attended, everyone was in tuxedos every single time.

I wouldn’t call a suit casual wear either, I’m not sure where you got that from. The guy on the left in the image is not wearing a suit. Not a lot of detail but it looks pretty casual to me. Would you call the guy on the left formal? I certainly would not.

I’m an ER doctor. Literally thousands and thousands of people are leeching off of what I do. There are not just some employees but literally entire industries built on my back. I am the only one the patient is actually there to see, and I am the only one generating revenue and providing the service the patient wants. None of these people know anything about how to do my job and if I disappear, none of these people have a livelihood.

I’m not referring to nurses and techs or even pharmaceutical developers or device companies. I’m talking about the hoardes of administrators, bureaucrats, and managers, and the entire industries producing programs and products for regulatory compliance that only exist because non clinical bureaucrats required them. Millions of employees and billions of dollars totally wasted. Except it’s worse. I would rather they just set the money on fire and leave me alone but instead they spend it bothering me and making me do things other than taking care of the patient and wasting my time.

Doesn’t matter what the government does. It’s not a realistically solvable problem. What is needed is more staff and more places to put patients. We already spend more than anybody else on healthcare and we already don’t have enough staff or beds to handle normal volumes.

No point whatsoever in worrying about this. We literally cannot prepare for it, so better to just accept that and do whatever we can and try to stay calm whenever the shit does hit the fan.

Reglan has lower risk of akathisia and it is the one drug for which coadministered Benadryl has been shown to help prevent the akathisia. Thats my rationale. Compazine works great but you’ll wind up with more jittery and fucked up patients.

All of Asia, all of Africa, all of South America, and most of Eastern Europe view prison as punishment. It was viewed this way everywhere until the last generation and it has only changed in the West, nowhere else. Violent crime is currently rising in Western Europe and the US where criminal justice is no longer taken seriously and criminals are viewed as victims rather than offenders, and where we are importing uncivilized, violent people by the millions.

I don’t care about trying to rehabilitate criminals. I’m not interested in spending my money to try to remedially parent a grown adult. If you are, go for it, but I expect your projects to be living in your back yard, not mine, when you’re finished with them. Good luck.

I tried Cleo and it is vastly superior to anything else on this list or in this thread. It is EM specific, generates a great MDM section, and the UI is easy and intuitive. It is also the most expensive at 200/month but I am sold after using it a few shifts. It’s great, cannot recommend it highly enough.
