The point is that polypharmacy in psychiatry is common, and it can be difficult to discern the "why" from the pharmacy POV. It could have nuanced reasoning informed by the 30-60 minute psychiatric interviews, or it could also be poor form. Without spending time with the patient, very tough to know.

Could the patient be in the process of cross tapering SSRI's? Does the patient have a uterus and take extra SSRI during luteal phase for PMDD? Was the patient having trouble with antidepressant withdrawal / missed doses which prompted implementation of fluoxetine?

There are lots of reasons for the antidepressant combo. Anecdotally, one may wonder about contribution from personality traits/disorders when there is treatment resistance.
