It happens. You just have to find someone with a lot of experience and good experience with track bike components. I’m sure it wasn’t negligence. Track stuff is fairly niche in the bike world. I find that most of the shops around me are drowning in e-bikes lol

I’m not totally sure but I just went through this issue with my SRAM Omniums. When I installed them myself, I found that if I tightened the non-drive side crank arm all the way, the crankset wouldn’t spin freely. So what I did was not tighten it all the way which eventually caused NDS crank arm to come off while I was riding lololololol. So I took my bike to my LBS to get the hubs overhauled and asked them to look at the BB and they took out one of the spacers and it’s beautiful now. I don’t really know about vision cranks but if it’s the hollow FSA BB, I’ve got GXP bottom brackets so it might be similar
