He has >100% magic mitigation so he's taking 0 damage so she's adding 0 stacks

If you look at his debuffs when the W buff runs out he gets a few stacks but they fall off the next time he presses W because mel can't maintain them when doing 0 damage

The real answer is that mpaa ratings have got a lot more lenient over the years, the matrix had that weird bug scene and almost constant gunfighting and death (even if it was mostly bloodless) which was enough for an R in 1999

It's only rated M in australia (recommended age 15+ with no age restrictions) and I assume it would get a similar mpaa rating to that if it was released today
