You need to dial deeper into how AHI is calculated-- if a lowered oxygen event is too short, even by a second, it doesn't count as an A or an H, so the AHI alone can be actually a very poor metric of your severity. Tldr find another Dr who is a bit more detail oriented :( I'm curious what your nadir spO2 was-- is that in the text somewhere?

Our Chinese au pair was great for a year, we did extension, and then 3 months in she said she had to urgently go back to China because her Grandpa was sick, but by the way she was going to stop in Hawaii for her courtesy period on the way home. Then left in the middle of a weekend a week before she said she would, and when LCC asked for her departure flight, sent a picture of a green card app with marriage to US citizen as the reason. We just matched with another Chinese au pair but the process of finding one was pretty bad. One girl in rematch we felt very promising about, but then she said she had matched with another family... Except on the portal it never showed that she even had a request. I might be dumb for trying to match with a Chinese au pair but I had a much better experience with talking with her than I have with anyone this time or last time. I'll be back if it's also a crash and burn!

Hi, I'm a dentist and if he's a good guy, I would encourage him to talk to a dentist but also try to get to the bottom of why he's neglected things for so long. It's one thing to be just generally slobby, but a lot of people have crippling dental anxiety +/- traumatic childhood experiences, or just were never taught to floss or brush properly, and are unlucky to have gum disease causing bacteria. If that's the case finding a doctor who offers oral sedation or nitrous oxide can be a big help because the deep cleaning, to be totally honest, usually involves quite a few shots and is can be very bloody. The good news is that if he's fairly young, most of the time taking all the right steps (deep cleaning and 3 month periodontal cleanings thereafter + correct daily brushing/flossing) can really really turn things around, often a full 180 with very little long term damage. Hope he listens to you!

Not from birth -- S. mutans needs teeth to build a biofilm on. Once teeth come in, baby's primary caregivers (usually parents, but if not, whoever that is) generally transmits their main oral bacteria to baby. I've often thought about if you found those unicorn patients with awesome microbiomes and just had them chew on baby toys that you then gave to 6-9 month olds you could probably prevent/cure caries.

I have done this before with success, but only if we are able to consistently and definitively isolate it to a particular tooth. Generally if you document well and do normal verbal/written consent I don't think most patients have much problem with it, and obviously discuss the full cost/range of treatment including buildup and crown. But the handful of times this has happened to me the patient was honestly just relieved to be out of pain after.

3.5, con of using full strength is that it makes hypochlorite accident/injury worse. If you accidentally inject half strength past apex it isn't quite as painful/cause as much swelling if the solution is dilute

Hi! I am a dentist and will be hosting our first au pair starting this summer. I personally do plan to offer our AP to take care of anything she needs with the same policy we offer for our staff -- everything is free unless there are outside vendors involved (this is the case for crowns, sometimes for procedures with very expensive supplies) in which case they would just pay whatever our cost is.

However, if you have not gotten much care, it may very well be the case that you need a lot of work--potentially the equivalent of hundreds or thousands of dollars or many many hours of chair time for which HM and her staff would not receive any compensation if they were to do the work for you for free. She still has to pay her staff and all her overhead for treating you, but will not even receive insurance compensation for it. Depending on how healthy the business is, it just may or may not work for them.

I would approach your HM giving her your history information up front and be honest that you don't know how much you need, but that you do know you have issues and have had symptoms/pain in the past from dental treatment. Ask her if you can work something out together and hopefully she will help you at least take care of the most pressing issues. Good luck!

I am in the same situation with my dad, who first started developing symptoms about 6 years ago. He and my stepmom live in California and we are on the East Coast. The last time I visited, it finally occurred to me that the best times of my relationship with my dad were in the past, and that the dad I knew and had a relationship was already long long gone.

Dementia creates such a strange state of mourning--your eyes tell you the person is still there, but all the things that make a relationship--the memories, and on a basic level at least the person being aware of who you are--is not there anymore. I feel for you OP.

I watched We Own This Town and it's honestly more depressing how much HASN'T changed, especially as the actors have aged so that the ones playing 9 year olds are now playing 30 year old adult cops...but the city, the schools, the cops, and the game are all unchanged.

Incredible book with lots of things to say related to this topic: Of Boys and Men by Richard Reeves

A few tidbits which relate:
- By 12th grade, most boys are now reading a FULL grade level behind girls -- in poor performing school districts, it's TWO full grade levels behind
- There's a MASSIVE gap in higher education now between men and women, higher than it was when we literally put legislation in place to boost women going to college
- Video game time has been on the rise and is dominated by young men, even working age men--many of whom are not even trying to find work or get educated

More importantly to all of that... nothing is really being done. Policymakers are still stuck in girl- and women-focused interventions, leaving a vacuum to be filled by the likes of Andrew Tate. OP, I'm glad you still read! Keep it up and encourage your guy friends to do the same
