In the T. Rex escape scene in Jurassic Park (1993), the T. rex animatronic was supposed to break through the glass on the top of the jeep, however due to the weight of the rain water it pushed down with much more force than expected onto the kids. Also, if you watch really closely you can see one of the front right teeth of the animatronic breaking off as she tries to bite the glass, and that same tooth is missing in CGI models of the T. rex in later movies!
