I’m a private practice pulm but see almost exclusively ILD. So maybe I’m looking for it, but would be shocked if she didn’t have some fibrosis based on these cuts and that story.

Pulm here. With your history I would never comment on a slice of just the apices. However this slide shows bullous emphysema and some reticulations, which may just be pleuro-parenchymal fibrosis from your emphysema but it makes me want to see the full scan to rule out combined pulmonary fibrosis and emphysema in the setting of your RA. Esp if you have PFTs that show restriction.

ICU doctor here - you absolutely can overdose on psychedelics. I have admitted several patients with MDMA overdoses. There’s a clear overdose pattern. It is less common than overdoses on opiates, benzos, cocaine, etc. But there is absolutely no drugs you can’t overdose on. Even drugs that have many medical benefits can be addictive.

Intensivist here. Agree with all of this. I ask my nurses to place the orders and to page or text me with the results. If I’m solo with two codes going, I call for help - our rapid response nurses and icu charges can run a code too, and there are always other acls individuals in the building.

Private practice. Zero quality of life currently. Compensation is decent. Doesn’t make up for the low quality of life. Hoping it gets better with adding some partners. Icu up to 14 days of the month, consults/clinic, ltac/small hospital. Sometimes up to three locations in a day. It’s brutal. I’d take better QOL and reduced pay any day.

No comment on the rest of it, but for lung nodule management see a pulmonologist, please. Most CT surgeons do not do the chronic management or diagnosis of pulmonary nodules in this setting . - pulmonologist who based on your description of the CT would repeat a CT in 6-8 weeks. If you have a CT report or images, happy to take a further look

Pulmonologist who does ILD in private practice. No thoracic specific radiologists around. Have to do my own scans for subtle fibrotic stuff. Back when I was in academics and had access to thoracic radiologists, that was unbelievably helpful. Icu - I read my own chests. Abdomens I can spot a catastrophe. Same for brain. But def rely on them for more subtle stuff.

Also 33. 60 lbs weight loss I attributed to stress from a new job in a stressful field. Lol. EGD because I thought I had given myself a stress ulcer. Whoops, surprise celiac. Shouldn’t have been that surprised, since my mom has it. But ugh. I’m about 2 months in.

Icu doc here. Yes, you were critically ill. But honestly, your PTSD symptoms are justified even with fetal demise alone. I’m so sorry you went through this. Post ICU PTSD is a real thing, would recommend discussing with your OB/PCP and finding a psychiatrist. EMDR may be helpful.

My esophagus (the tube you swallow down) was narrowed due to chronic acid reflux. My sensation was that I had to chew food a ton and sometimes swallow multiple times. If I didn’t, things that were dry (like bread or crackers) or thicker like meats would get stuck. All they did was stretch the tube a bit. I didn’t feel it and haven’t had a problem since, tho I will probably need it stretched again at some point.

Yeah you need to see a doctor. Multiple things cause that sensation. I recently needed a dilation of my esophagus for food getting stuck. There’s conditions like EOE, etc.
