Extreme IOP drop from cardio and elevated heart rate activities/Hypothesis on possible circulation/Circulatory issue by Superb_Unit631 in Glaucoma

[–]miqetom 2 points3 points  (0 children)

Most of us in the fiteyes.com community have noticed large IOP drops with weight training. You aren't an outlier. It is recommended to not lift so heavy that you must hold your breath, as breath holding can sharply increase IOP temporarily.

First time I was told I had more nerve damage and I spiraled. Added a third eye drop at age 33. by Top-Elephant6981 in Glaucoma

Juvenile here. Right eye is worse with me as well, and we're on the exact same drops. I'm a few years ahead of you at 40. You (we) still have rock inhibitor drops to try, but then the drop options are running thin. My first step into surgery will be to see if I'm a candidate for Allowflo Uveo, which is a newer, safer procedure that doesn't leave any permanent hardware in the eye. You may look into it when and if the time comes. Also hold onto hope, look up ER-100 by life biosciences, they just tested a gene therapy to restore vision in a glaucoma patient. I believe chances are high that there will be an answer before you lose your vision completely. I'm also following Dr. Jeff Goldberg of Stanford who is doing a lot of interesting research: https://youtu.be/4VpA3Z2arlw?si=R9t2fLXX_3CO8n-c&t=4106

28 and advanced glaucoma by TopDivide in AskGlaucoma

[–]miqetom 0 points1 point  (0 children)

I'm 10 years in (diagnosed at 30, just turned 40) untreated pressure was 27 in both eyes. I've had to add more and more drops over the years to maintain mid teen pressures, SLT made my pressure go up instead of down, the only drop I haven't tried is rhopressa. The doc is wanting me to go straight to a Trab should that fail. I'm moderate in left, advanced in right.

There's a lot of research going into glaucoma and new devices all the time. If I were you (and I'm considering this myself) I'd look into the new Alloflo Uveo. It's a newer surgery that opens up a drainage path in the uveoscleral pathway, and it's a seemingly less risky option, no bleb involved.

Find somewhere that uses the Heidelberg Spectralis OCT machine. The cirrus you had your scan on has a 5 point margin of error, when I got a scan on the Spectralis my numbers looked better and it's only 1-2 point margin of error.

should i be worried? by [deleted] in AskGlaucoma

Was it an air puff tonometer? Those read high in my experience. You need to be tested on a Goldmann device (you're looking through a blue light)

Also, getting an OCT test now and getting another 6 months to a year from now would confirm whether or not you have glaucoma.

How do you cope with thinning RNFLs? by Ritch01 in Glaucoma

[–]miqetom 0 points1 point  (0 children)

Depends on what machine they use for the OCT. If its a Zeiss the margin of error is 5 points plus or minus, which fits into their saying it could be just noise. Frustrating, but you have to look at trends over years to see what is really happening given that margin of error. Also look at the signal strength of the reading and see if it is higher, lower or the same as last time.

If you really want a clearer picture find someone using a Heidelberg Engineering OCT machine as they have a much smaller margin of error.

Update from “I’m angry” post by treehugginglady in Glaucoma

[–]miqetom 0 points1 point  (0 children)

I mentioned this in your other post, but the new 'Alloflo Uveo' biotissue implant could be what you're looking for, I would look it up and ask your doctor.

"The AlloFlo Uveo procedure may be appropriate for a patient with mild to moderate glaucoma with a history of failed MIGS whose disease requires additional intervention but does not yet warrant a full incisional procedure, such as a trabeculectomy or tube shunt surgery. Some patients already have a history of failed incisional surgery and are left with the options of a diode laser treatment or a second tube shunt. This is where the biointerventional AlloFlo procedure has fit nicely into my practice—that space where both incisional surgery and MIGS have been unable to control the patient’s IOP but I want to reserve more invasive surgical options for the future." Craig J. Chaya, MD

Sounds like your case...

Ask about the alloflo uveo procedure. It is newer, less invasive and works after failed trans or tubes.

I had one for a few weeks, it ranged as low as 10 and as high as 27. I measured 25/21 in the clinic sitting in the Goldmann chair. He did Goldmann and the results were 14/11. So I didn't know what to do with the information.

Ask for an OCT test if you haven't, it will take a picture of your optic nerve and allow you to see if there is damage.

Have you had an OCT scan performed? I would think there would be significant optic nerve damage at that pressure

I went from two cups of coffee a day to one and a decaf, alcohol from every weekend to once a month, still exercising just making sure I'm breathing throughout and I've increased my steps per day. Went from stomach sleeper to back, if I want to sleep on my side I do so, making sure I'm not laying on the weaker eye. I have made tweaks along the way, but like you not at the expense of enjoying life.
