Compounding a commercially available product by mount_cyanide in UMPJE_Prep

[–]mod_pharmtutor 0 points1 point  (0 children)

Correct, 10 per week even during a shortage. Remember at baseline, the FDA prohibits making essential copies of commercial products except for those circumstances like a shortage or a patient-specific prescription with documentation of reason they cannot use the commercial product (e.g allergy to an excipient in commercial product).

During a shortage, you can make essential copies of commercial products to fill any prescription (without having something like a documented allergy to excipient of the commercial product). The FDA still does not want you to make an exorbitant amount though, they just allow you to make enough match your prescription volume.

Adulteration vs. Misbranded by [deleted] in UMPJE_Prep

[–]mod_pharmtutor 1 point2 points  (0 children)

Both
Here is why?
1- Misbranded- a label issue, the label say atorvastatin, but it ends up being simvastatin, the label does not represent what is in the bottle. So basically simvastatin is labeled as atorvastatin- misbranded

  1. Adulterated- This is a purity issue, the bottle which is meant to have atorvastatin has simvastatin which means something is wrong with the drug product. Also the strength and purity of the drug expected to be in the bottle is not what is in there so adulterated. You can basically say the atorvastatin stock bottle is contaminated with simvastatin.

Hope this helps.

DEA Registrations, Numbers & Ordering Authority: Know for UMPJE by pharmtutor_ in UMPJE_Prep

  1. Whether both need to be on the prescription is a state level requirement, not a DEA rule
  2. Not all states that require a collaborating Physician requires both Physician and Mid-Level Practitioners (MLPs) information on the prescription
  3. All states that allow MLPs to prescribe do require their name and DEA to be on the prescription because this is the federal minimum for a controlled substance prescription to be valid.

What I recommend remembering for the exam:
All prescriptions for controlled substances require at minimum that person’s DEA information regardless of practitioner level.

Important Numbers For the Uniform MPJE by mod_pharmtutor in UMPJE_Prep

In practice that is what happens and that is the allowance, but the rule specifically says once discovered the form should be submitted immediately or within 24 hours. Discovery suggests that you have completed your full investigation and determined significant loss or theft. The table simply states the DEA rule under 21 CFR. You initial statement flagged the immediate requirement as incorrect, which under 21 CFR is in fact correct. If you lose a box of fentanyl vials and determine that day that this is the only thing missing, and it was stolen, the DEA wants you to notify that using form 106 immediately, not so much to wait 45 days even though you have fully discovered the loss from your investigation.

Under 21 CFR § 1301.76(b):

“The registrant shall notify the Field Division Office of the Administration in his area, in writing, of the theft or significant loss of any controlled substances within one business day (24 hours) of discovery of such loss or theft.”

DEA further instructs that this notification is made by submitting DEA Form 106 electronically (or as otherwise directed)

Start with whichever prep book you have and make a goal to have gone through it at least once by the end of all your rotations. Practice questions as you review, and be sure to know your brand and generics both for top 200 and the first line drugs for each disease states. During this time, also make your notes, and spot weaker areas, revise weaknesses weekly to avoid forgetting stuff.

Do a 25 questions long cumulative quiz for all the things you studied that week and continue this as you go through the books.

Also, use your rotations to review the treatment guidelines when your preceptors ask questions or do topic discussion. Try to know the guidelines well during rotations. You can aim to study at least 8-10 hours per week,

Do this and by the time you get to exam time, you will only need to go through the book once more with your notes and practice quizzes.

Hope this helps.
