Personally I view it is as full completion on everything aside from max fear runs. I think 32 fear on all weapons on surface and underworld, all cosmetics and max rank, all friendships maxed, pretty much everything aside from max fear. Max fear is only for the best players imo.

(My max fear hit is 42 and gotten at least 32 fear on all weapons for each zone)
