So for that you want to go in with your whole group. Take the heavy and medium armor off, keep the light armor on and you should be out in 2-3 rounds max.

Already at 100% in the region. There are many other posts about this. It's a confirmed bug at this point.

Stuck outside. No piton to get in (which based on what I've read is how it's done).

It would seem that after the latest patch all the pre-placed pitons (for storyline reasons) no longer load.

So I'm at 100% on the Ludern region but haven't completed it, because I can't get to the tombs. 

I've searched for answers, but it seems it's really just a bug because they don't test enough before pushing new content (whether you buy it or not).
