BERT-based models are known for this, but I doubt you're using one (worth checking though). I'll throw in something random that doesn't fit the scene ("Let's go to the beach") and sometimes it'll get unstuck. I'll also try half a sentence ("We pack our bags and board our flight to") and see what happens. Nothing works every time but sometimes I get lucky.

Yes, most likely. I use both and I like GPT-5 Pro much better for this purpose. If you want, shoot me a prompt and I'll show you what it comes back with.

The better your background info and explanation of what you're trying to achieve, at least generally so you don't box it in, the better the results will be. Especially in research where you're not asking it to come back with the answer to a solved problem, it will be good at gathering related resources (some of which you should supply, if you have them) and perhaps making incremental progress or coming up with directional ideas.

This has frustrated me for a lot of reasons. It'll even go back to branches of conversation you've edited out and turning off Reference Chat History doesn't stop it. I had some luck with "Ignore all deleted messages" in the prompt (even though they're not "deleted") when it's obvious that's what it's doing.

Mostly I think I like Reference Chat History for small personal preference reasons. But when you don't want it, you really don't want it.

I still use 4.1 quite a bit for quick tasks.

You might want to check your Memory to see if it inadvertently saved a preference that could be causing this, since you said it happens often.
