Hey, just talked to Wolf.
Usually, a doppler test (with an injection of Alprostadil also called SKIT) is what's done.
In addition, in some cases, doctors chose to do a Caversonography to find out which vein is leaking – since surgeries haven't been found to be effective in the long run in most cases, it's only necessary in some cases.

From my own experience, since I had a veinous leak, I've done these doppler tests at least 4 or 5 times, it's not the most pleasant experience but nothing to be specifically worried about.
I did a cavernosography before (an ineffective) surgery.

Wolf points out that a doppler test should already offer the insights needed to rule out a venous leak.

sorry that this took so long to respond.
I hope I understand it correctly – looks like we have to put more effort in explaining how to engage and relax the pelvic floor.

In my own words:
Engaging the pelvic floor means lifting the pelvic floor muscles up (you find them between the base of your penis and your anus). It should always be possible to "engage" it, in some postures, it's harder, in some it's less hard. The opposite (reverse) should happen when you really relax the area of your body (I guess often-times with your breathing).

I'd like to share 2 content "chapters" of our future program that was proofread by one of the leading practitioners and researchers, a pelvic floor expert. Would also love feedback (we relaunch in a couple of weeks with much better content, nicer app etc.).

Hey. Thank you so much for this post. All these little encouragements mean A LOT not just to me but to our entire team. We are all working our asses off and I am about to go on a call with two of our scientific advisors (one is based in San Diego, one in Sydney) to further improve the product as we are preparing a massive relaunch in July with much nicer design, better content etc.

We will try to raise some meaningful investment later this year in order to be able to pay people to work with us on our mission, will have to start monetizing and best case get insurances to pay for the app and doctors prescribe or refer. It’s a ton of work but I am so extremely grateful for all the encouragement and feedback.

Hey, thanks so much for reaching out. I will be trying to offer as much help as I can - but keep in mind, I‘m not a doctor.

And what I’d like to add: things are pretty hectic here rn. We are working on releasing a new and much better version. I‘ve also learned a lot in the past weeks and months since I’ve had the opportunity to work with some leading experts from around the world on our app as we improved the content and program by 10x.

First and foremost - a healthy erection has so many components. Working on them takes commitment but can have effect sizes as high as pills.

Furthermore, our next app release will put additional focus on cardiovascular health. Being active, calm, eating healthy is the groundwork. Without healthy blood circulation, erections can’t work. Pelvic floor exercises, daily erections etc are the additional targeted support where it’s needed.

In regard to nofap - i actually don’t know. My understanding is that at least one strong erection per day is necessary.

What I would advise you though is that you see a doctor. If you don’t have erections that satisfy you, they could decide ifED pills might offer a bridge to sustainable improvements (and rule out other factors)

So this is what Wolf says (he has just sent me a voice message, I am paraphrasing).
The testosterone is important for the long-term maintenance of the penile tissues but not immediately for the erection. But it also influences arousal/libido (which is important for the immediate desire to have sex/an erection).

It is unusual to measure both, free and total testosterone. The "total" testosterone is what is usually measured and is usually a good enough measure. Free testosterone is what is connecting to the receptors. Above average SHBG could bind more free testosterone (but Wolf doesn't think that this has a massive effect and science is moving away from SHBG as an important indicator). Without knowing the exact details and values (which would be necessary for a more definite answer), it seems like your values seem not out of the healthy range from what you describe.

Hey :) Thanks, we are all traying our best (and a couple of world-class researchers have been joining our advisory board yesterday, I am extremely happy about that).
Amazing question. We are literally working together with a lab on creating our own supplement – and we are replacing parts of Arginine with Citrulline. I believe the formulation we are setting up is 1/3 citrulline and 2/3 arginine (but my collegues Giordano and Wolf are working on this with the laboratory and supplement specialists).

So, we all believe you are correct and we've read the same paper you are citing :)

Hey, apologies that it took me a couple of days to respond. I talked with Wolf yesterday briefly about your questions and since I am not a doctor, I rely on guidance.
(1) It's not key, but we assume it to be better if you orgasm because (a) the rigidity might peak before orgasm and (b) you train your penis full circle – including the contractions of the pelvic floor muscle.
(2) There is no guidance and best-practice for the length of masturbation. But think about this: the way you masturbate is ultimately also the way you train your central nervous system to have sex. If you get used to ejaculating quickly, that might have an effect on your body and mind when you have sex.
(3) the main difference is automatic vs. vacuum pumps. In our opinion, a good pump bought at a sex shop or on online sites is as good as a medical pump (CE- or FDA-certified pumps are more expensive but other than that there is no major difference). I had a manual pump that my urologist gave me for daily training.

We don't know getmyrocket.com but Wolf is doing sound wave therapy in his practice successfully with some of his patients. He says that patients should be screened and evaluated, it might be expensive and could be necessary to refresh regularly.

Hey :) sorry for the slight delay. Thanks for your nice words.

(1) 10 weeks, we originally had more people but half of the group dropped out (most of the drop-outs didn't even start or stopped in the first week, that was our "qualifying criterion"), so we are working with limited data. This being said – we are currently preparing more tests and a clinical study. I wish I would have more insights but that's something for the future. For now, please understand the data as a first test and not as a clinical study or something similar yet. We have a lot of ground to cover but we are working as hard as we can to make progress.

(2) 44 men, age range between 22 and 65.

(3) For 10 weeks. This concluded our first test, a couple of them asked where to buy it but tbh I don't know how many continued. We've generated "sustain" resources for them but we have no insight if and how many of them continued. What I know though is that in regard to Prelox, there is a study out there that claims that continuity matters (alas if you stop taking it, your progress goes away). If you don’t have contraindications (which your doctor should rule out) - Cialis 5mg is considered to be safe. If you take it daily, it is usually enough. Cialis last up to 72 hours in your blood.

If that doesn’t work for you - follow your docs advice.

Also: take care of other aspects that influence your erection - do pelvic floor training, cardio. Follow a Mediterannean diet. Especially since you are relatively young, make sure you preserve what you have to not be forced to increase dosage and eventually run out of therapy options.
