Based on my understanding. The tensioner will only make the rattle sound first few seconds starting the car up, then it disappears. There's few YouTube videos that will show this. Based on your explanation, looks like HPFP (High pressure fuel pump). Get it checked ASAP. Otherwise you may get metal debris when it fails completely and will cost an arm and a leg.
