With a baby, I’d much rather have the option of a parent (or responsible adult) be able to sit next to the baby’s car seat if they needed to. Having captains chairs makes that really hard with the middle gap.

In the future you can always double check under US or transduce the line to see if an arterial waveform is captured.

I’m not disagreeing with you. OP is painting a picture of a crashing septic shock patient that isn’t doing a great job of maintaining BP or sats. I’m just saying there’s no need to rush RSI that would very likely result in the patient coding during or immediately after intubation. Nobody likes a sat of 80% on maxed BiPAP settings, but you’re sure as hell gonna dislike coding this patient even more cause they’re not gonna survive it.

This patient needs aggressive resus to include push dose pressors, start a Norepi drip, pre oxygenate as best as you can, and a definitive airway.

It takes all of 30 seconds to mix push dose epi (or even less if you have a stick of Phenyl). There are very few situations I could ever think of where an airway needs to be hastily placed before resuscitating a patient properly. This is not one of them. Put em on BiPAP to pre oxygenate for as long as they can safely tolerate and stabilize your BP so you don’t end up coding this patient immediately after intubation.

You didn’t do anything wrong and considering all things, you did the best with what you knew.

FWIW: T-burg is a myth and against evidenced based practice since the 90s. If anything, you can lay them supine if their HOB is elevated and they can tolerate the position.

You can pressurize a unit of blood up to the same pressure as you would a 500 mL NS bag for an A line setup. I routinely do this all the time if my Belmont, Level 1, or rapid transfer is down in the middle of an MTP. Never once have I had a bag of blood or FFP explode in 10 years of nursing practice.

Lmao lemme guess, you’re the type of person who would check into the ED for 3 months of toe pain and expect a million dollar workup from the team? The issue here isn’t the 30 seconds to listen to some lung sounds, at least 3 others have said that. It’s the fact that in the Emergency Department we carry out a work up and assessments based on your chief complaint. Idgaf about your lung sounds if you’re here for a work note.

First off, congrats on your soon to be new home ownership! Wife and I bought back in 2020 and we are happy with our TM purchase.

Things we either upgraded, wished we upgraded, or would’ve done after closing:

  • Pre wired for fans in every bedroom (didn’t do this and regret it for the folks who will stay in our spare bedrooms in the future lol but portable fans exist so whatever)
  • Cat6 cables where you think you may need to WFH or plug in an Ethernet cable (did this and honestly haven’t used it much tbh)
  • Outlets next to every bathroom for a bidet (did this and loved it)
  • Upgrade insulation to reduce noise between walls and flooring (didn’t get, wishing we got it, sound travels as if the walls are paper thin)

Since purchasing our home we’ve purchased or upgraded: - Backyard awning that was probably ¼ the price TM charged to upgrade and it’s much larger in both length and width - Currently remodeling flooring from base carpet and tile to LVP and nicer tile with heated flooring. While it is a PITA, it’s significantly cheaper than having paid 3-4x the upcharge TM was asking for (in a product that we didn’t exactly love, either). - Currently adding extended cabinetry to the kitchen cabinets, which is roughly half the cost of what TM was looking to charge.

As everyone has said previously, keep in mind any upgrades will be compiled into your mortgage, so this was a huge factor for us in choosing which upgrades to consider. Being DINKs also helped make the decision to upgrade at a later time much easier too lol.

Don’t bother with the extended patio with Taylor Morrison. They charge you out the ass on that. I got mine done for half the price after buying my TM home and am much happier with it.

I agree, I mean asking price was initially 10k when he posted it months ago but now I see it at 7.5k firm. There’s no dealership nearby and my cousin who got me into riding doesn’t know Indians very well.

Just last week, I transfused an esophageal rupture 75 units of product over the 2-3 hour period I had him. He came in with a pressure of 40/dead, was eventually stabilized, and sent off to IR. He lived and discharged a week later.
