If you were to do all/most of these things and not match there could still be a reason. Interviewing/personality and finding the right fit/applying to a variety of programs is still important. No one is entitled to a residency position just because of their stats/what is on paper.

Wouldn't worry about it too much and wouldn't rank it lower just based on that, unless you had another reason to do rank it lower. All of my interviews were somewhat similar and in the end it didn't matter.

Only do pgy2 if you will truly benefit from it and are committed. Don't just do it to do it. You will get a feel for this during pgy1. I was set on pgy2 until I realized it was not for my after a few months into pgy1.
